William S. Middleton Memorial Veterans Hospital
Berook Addisu, Pharm.D., BCPS, is the Associate Chief of Pharmacy Service at the William S. Middleton Memorial Veterans Hospital in Madison, Wisconsin. In this role, Dr. Addisu assists the Chief of Pharmacy with planning, directing, and coordinating comprehensive pharmacy services to veterans residing in the south-central Wisconsin and northwestern Illinois regions. He participates in many facility committees and workgroups including the Pharmacy Practice Model Initiative Committee. He is also involved in the training of residents and serves as a Clinical Instructor for University of Wisconsin-Madison (UW-Madison) School of Pharmacy students. His current practice interests include acute care management and clinical development.
Dr. Addisu earned his Pharm.D. at the UW-Madison. He completed a PGY-1 Pharmacy Practice Residency at the VA Tennessee Valley Healthcare System in Nashville and Murfreesboro, TN. He then completed a PGY-2 Health-System Pharmacy Administration Residency at his current facility. In addition to ASHP, he is also a member of the Pharmacy Society of Wisconsin and a board member of the UW-Madison Pharmacy Alumni Association. Dr. Addisu is a Board Certified Pharmacotherapy Specialist.

Andrew Wilcox is Chief of Pharmacy Services at the William S. Middleton Memorial Veterans Hospital in Madison, WI. He received his Doctor of Pharmacy degree from the UW School of Pharmacy in 2001 and completed his residency training in Primary Care at the Madison VA Hospital in 2002. Following residency he worked as a Clinical Pharmacy Specialist at the VA Primary Care Clinic in Rockford, IL specializing in chronic disease state management and anticoagulation. Throughout his career he also has had a strong interest in leadership and management and became Assistant Chief of Pharmacy at the Madison VA in 2008. Dr. Wilcox is a graduate of the 2009 ASHP Foundations Pharmacy Leadership Academy. He is currently an appointed member of ASHPs Section of Pharmacy Practice Managers Advisory Group on Manager Development and PSWs PPMI Leadership Team.
