Ernest Dole,
PharmD, BCPS, Clinical Pharmacist,
Pain Consultation & Treatment Clinic, University of New Mexico Hospitals
Dr. Dole is Clinical Staff Pharmacist III at University of New Mexico Hospitals (UNMH), Department of Clinical Pharmacy and Clinical Associate Professor at University of New Mexico Health Sciences College of Pharmacy. Dr. Dole manages ambulatory care patients with chronic noncancer pain. He received his B.S. in Pharmacy from Colorado, with honors, and his Doctor of Pharmacy degree from Tennessee. He completed a clinical pharmacy practice residency at the Memphis VAMC. Dr. Dole was the ninth Pharmacist Clinician in New Mexico in 1996, was recognized as an ASHP fellow in 1997, passed the Board of Pharmaceutical Specialties Pharmacotherapy exam in 1997, and Certified Diabetes Educator exam in 2000. Dr. Dole has twice been nominated for the NMSHP Pharmacist of the Year. He has been the Chair of American Society of Health-System Pharmacists Section of Home, Ambulatory and Chronic Care Practitioners and has served as a member on ASHP Councils and Task Forces. He has been president of the New Mexico Society of Health-System Pharmacists. Dr. Dole received the Volunteer Faculty of the Year Award and Preceptor of the Year Award from University of New Mexico Health Sciences College of Pharmacy in 2009. Current research is obtaining billing privileges for pharmacists, effect of high opiate doses on testosterone levels and the effect of opiates on sleep apnea.

Virginia Ghafoor,
PharmD, Clinical Pharmacy Specialist - Pain Management,
University of Minnesota Medical Center
Dr. Ghafoor has worked as a clinical pharmacy specialist in pain management and palliative care for 20 years. In her current position, she is responsible for coordinating the pain medication stewardship and narcotic safety programs at the University of Minnesota Medical Center. Last year, she received a grant from ASHP to develop a PGY-1 residency in pain management transitions of care which is a new, innovatiave program. Dr. Ghafoor has several publications in peer-reviewed journals on pain topics including intrathecal pumps, palliative sedation and patient controlled analgesia. She has authored chapters on pain management in Applied Therapeutics, ASHP's new Smart Infusion Pumps book and the American Society of Pain Management Nurses Core Curriculum. Over her career she has been an invited speaker to more than 30 professional meetings to present on pain management. In addition, she is the instructor for didactic courses in pain management at the University of Minnesota College of Pharmacy as well as a preceptor for students and residents.

Anthony Pazanese,
PharmD, Clinical Pharmacy Specialist,
Lakeland Regional Medical Center
Anthony Pazanese is originally from Tampa, FL. He earned his Doctorate of Pharmacy from Palm Beach Atlantic University College of Pharmacy in 2010. Dr. Pazanese completed a Specialty Pharmacy Residency in Pain Management and Palliative Care at Lakeland Regional Medical Center, where he currently practices as a Clinical Pharmacy Specialist. He has experience in treating both acute and chronic pain and managing patients with co-morbid psychiatric and substance abuse disorders. Dr. Pazanese has spoken within the institution, as well as regionally, on various disease states and opioid pharmacology.
Dr. Pazanese serves as a preceptor for pharmacy students and is a Clinical Assistant Professor at the University of Florida and Palm Beach Atlantic University. He is also co-director of Specialty Pharmacy Residency in Pain Management and Palliative Care at Lakeland Regional Medical Center, which is in the process of becoming ASHP accredited.

Barbara Petroff,
BS, MS, Performance Improvement Chair, Student Mentor,
Critical Care Systems
Barbara J. Petroff is currently chair of the Performance Improvement Committee and Student Mentor in the Wixom branch of Critical Care Systems, a national home infusion company. In addition she is an accreditation surveyor for the Accreditation Commission for Health Care and a principal at Shawler Petroff LLC. She also teaches healthcare management classes at University of Phoenix and is adjunct faculty for four pharmacy schools. She received her BS in pharmacy at Ohio State University and an MS in administration at Central Michigan University. She has worked in several pharmacy arenas including research, hospital, retail and the last 25+ years in home infusion.
Barbara has served ASHP in many capacities including committees, advisory groups, and as Director-at-Large of the Section of Home, Ambulatory and Chronic Care Practitioners. Currently she is the Chair of the Home Infusion Section Advisory Group of the Section of Ambulatory Practitioners. She also serves as Chair of the Standards Committee for the National Home Infusion Association.

- Determine appropriate drugs and devices for pain management that can be used across the continuum of care for intravenous, subcutaneous, epidural and intrathecal routes.
- Utilize practice models that integrate prescription monitoring programs and practice protocols for transitions of care.
