Dr Doyle Petrongolo is ambulatory pharmacist in the Outpatient Pharmacy at Massachusetts General Hospital. She received her BS in Pharmacy from Northeastern University and Doctorate of Pharmacy from the University of Rhode Island.
She began her pharmacy career at CVS then later went on to Tufts Medical Center where she worked in the Outpatient Pharmacy. Since 2005 Dr Doyle Petrongolo has worked at Masschusetts General Hospital Outpatient Pharmacy. From October 2007 to December 2008 she had been part of the ambulatory pharmacy pilot study working in a primary care office assisting in patient counseling, disease state management, and helping patients with medication access issues including Medicare Part D selection. Dr Doyle Petrongolo is also the pharmacist liaison for the Medicare Care Management Program
where she works with case managers and social workers to resolve medication problems in a high risk population. In 2010, the Care Management Program partnered with a statewide initiative to prevent re-avoidable admissions within this Medicare demographic. Since then Dr Doyle Petrongolo has been meeting with patients prior to discharge to review medications and assist in coordinating their prescriptions. By rectifying the many medication errors on the discharge medication lists, she was given a Patient Safety Star Award in 2011. More importantly, the information learned from this program has influenced medication safety practices within the hospital.
She has given many presentations on geriatric topics including: Polypharmacy, Medication Access issues, and Fall Prevention risk assessments and participated in several medication safety and discharge process improvement committees.

Joseph Dula,
PharmD, Regional Director of Clinical Services,
Pharmacy Systems, Inc.
Joseph Dula completed his undergraduate studies at Miami University (Oxford, OH) earning a Bachelor's degree in Chemistry. He then attended The Ohio State University
College of Pharmacy, graduating as a Doctor of Pharmacy.
Dr. Dula served as a Staff/Clinical Pharmacist for Berger Health System, Circleville, OH. While a member of Berger's healthcare team, he was responsible for dispensing as well
as daily clinical functions such as intervention monitoring, evaluating antibiotic therapy, and formulary optimization.
Dr. Dula now serves as a Regional Director of Clinical Services and is responsible for establishing and managing clinical programs consisting of pharmacokinetic services,
therapeutic reviews and interventions, medication use evaluations, and implementation of cost containment processes. To augment the management of these programs, Dr. Dula is
involved with many activities such as Pharmacy and Therapeutic Committee meeting preparation and presentation, development of newsletters, and provision of medication education to pharmacy, nursing and medical staff.
Dr. Dulas goal is to reduce and control drug costs, enhance hospital-wide patient safety and provide education to healthcare providers to enhance patient care.

Paul Glynn,
Pharm D, BCPS, CGP, Clinical Pharmacy Manager,
Abbott Northwestern Hospital
Dr. Glynn is a Clinical Pharmacy Manager at Abbott Northwestern Hospital in Minneapolis, MN. His responsibilities include oversight and management of clinical pharmacy activities for cardiovascular, neuro/ortho/spine, behavioral health, and infectious disease services. Along with these areas, Dr. Glynn's clinical interests include work with core measures and transitions of care. He is a 2000 graduate of North Dakota State University, achieved Board Certification in Pharmacotherapy in 2004, became a Certified Geriatric Pharmacist in 2007, and is a 2009 graduate of the ASHP Pharmacy Leadership Academy. Dr. Glynn is actively involved in several local and national organizations, including current service as President of the Central Minnesota Society of Health-System Pharmacists. He is pursuing a distance-based MHA via Simmons College in Boston, MA, with an anticipated graduation date of September 2013.

Steven Riddle,
PharmD, BCPS, Vice President of Clinical Affairs,
Pharmacy OneSource/Wolters Kluwer Health
Dr. Riddle serves as Vice President of Clinical Affairs for Pharmacy OneSource/Wolters Kluwer Health; a company that provides web-based technology solutions and evidence-based content to support optimal healthcare delivery. Steve is responsible for strategic planning and marketing and serves as liaison for the company to the healthcare profession.
Steve received his Bachelor of Pharmacy from Washington State University and his Doctorate of Pharmacy from the University of Washington. He is currently a Clinical Affiliate Professor at the UW School of Pharmacy and serves on the Washington State University College of Pharmacy Deans Advisory Council. Mr. Riddles specific professional interests are quality improvement, health care system reform, and wellness centered healthcare.
Dr. Riddle serves as the Chair for the Section of Ambulatory Care Practitioners for the American Society of Health-Systems Pharmacists. He currently acts as a representative for the ASHP on the Pharmacy Quality Alliance where he is editor of the PQA Quality Connection and serves on the Care Transitions Workgroup. Steve served for 3 years as inaugural Chair for the Washington State Pharmacy Associations Health Systems Academy. He is a member of ASHP, ACCP and WSPA and supports these associations goals of integrating and expanding pharmacy practice to improve patient care. Steve was named a Fellow of ASHP in 2009 and received distinguished leadership awards from ASHP and WPSA.

Richard Stambaugh,

St. Cloud VA Healthcare System
Dr. Stambaugh has been involved with ASHP and state affiliates since the 1970s. He currently serves the Section of Ambulatory Care Practitioners as Vice-Chair of the Section Advisory Group on Clinical Practice Advancement, and is Chief Pharmacist at the St. Cloud VA Healtcare System. VA service includes multiple positions thru the continuum of care including emergency response, hospital, ambulatory, home and long term care. In 1980, he completed residency at the Iowa City VA, and a Masters Degree in Pharmacy from the University of Iowa. Dr. Stambaugh completed B.S. and Pharm.D. degrees at the University of Minnesota. VA sites served have included Minneapolis, Milwaukee, Kansas City, and Baltimore before transferring home to the St Cloud VA Health Care System as Clinical Pharmacy Coordinator in 1988.

- Develop a plan to incorporate new practice models, programs, services and/or pharmacist interventions to reduce hospital readmissions in the inpatient, outpatient and transitional care settings.

- Identify how to gather return on investment data for reducing readmissions and creating sustainable services.
