Kelly Epplen,
PharmD, BCACP, Assistant Professor of Clinical Pharmacy Practice,
James L. Winkle College of Pharmacy, University of Cincinnati
Dr. Kelly Epplen is an Assistant Professor of Clinical Pharmacy Practice at the University of Cincinnati, James L. Winkle College of Pharmacy. Dr. Epplen received her Doctor of Pharmacy degree from the University of Cincinnati in 1997. She began her career at the Health Alliance of Greater Cincinnati where her primary role was the development and implementation of ambulatory clinical pharmacy services. She has been instrumental in developing pharmacist-managed anticoagulation clinics and pharmacotherapy clinics at University Health, Cincinnati, as well as St. Elizabeth Healthcare of Northern Kentucky. Her responsibilities include teaching and direct patient care. In addition, she is actively involved in residency training and has served as program director for both PGY1 and PGY2 pharmacy residency programs. She joined the University of Cincinnati faculty in 2009 and provides direct patient care at the Family Practice Center of St. Elizabeth Healthcare in Northern Kentucky. She is a faculty member for the ASHP/APhA Ambulatory Care BPS examination review course and has numerous publications in the area of ambulatory clinical pharmacy practice. She has co-authored the business plan development chapter in ASHPs recent publication: Building a Succsessful Ambulatory Practice. She obtained board certification as an ambulatory practitioner in October, 2011, and is a member of the Society for the Teachers of Family Medicine.

Seena Haines,
FASHP, BCACP, Professor and Dean for Faculty,
Palm Beach Atlantic University
Dr. Seena L. Haines is Professor and Associate Dean for Faculty at Palm Beach Atlantic Universitys School of Pharmacy. Dr. Haines completed an Ambulatory Care residency through Nova Southeastern University/Veterans Administration. Dr. Haines developed a replicable medication therapy management model, which resulted in the founding of four pharmacist-based, indigent care clinics in Palm Beach County. The Integrated Pharmacotherapy Services clinics received DSME ADA recognition as a single-provider/multi-site program and have received over $750,000 in grant funding to provide pharmacist-directed primary care to the underserved. In 2008, Dr. Haines became co-director of the Diabetes Education and Research Center. Dr. Haines established Palm Beach Atlantics first Pharmacy Practice residency serving as Residency Director (PGY-1). At PBAU, Haines teaches disease prevention/health promotion and case studies in therapeutics. Haines is an editorial advisory board member for pharmacists letter and diabetes spectrum. Dr. Haines is the Director at Large for the ASHP Section of Ambulatory Care Practitioners through 2012. Other professional achievements include: Board Certification in Ambulatory Care Practice (BCACP), AACP Innovation in Teaching Award 2010, Chair for the ASHP SAG on Reimbursement for Cognitive Services, inaugural AACP Self-Care SIG Chair, Certified Diabetes Educator (CDE), Board Certified in Advanced Diabetes Management (BC-ADM), 2009 Preceptor of Distinction, 2008 Hero in Medicine, and inaugural AACP Academic Leadership Fellow, Co-Editor andsteering committee member Non Prescription Medicine Academy.

Sandra Leal, PharmD, FAPhA, CDE is the Director of Clinical Pharmacy/Broadway (Special Populations) at El Rio Health Center in Tucson, Arizona.
Dr. Leal currently serves as Director-at-Large for the Section of Ambulatory Care Providers. In addition, Dr. Leal has been working to promote clinical pharmacy services nationally to improve patient safety and health outcomes as Co-Chair for HRSA's Patient Safety and Clinical Pharmacy Services Collaborative.

Betsy Shilliday,

UNC Internal Medicine & Eshelman School of Pharmacy
Betsy Bryant Shilliday, PharmD, CDE, CPP, is an Associate Clinical Professor with the UNC School of Medicine and UNC Eshelman School of Pharmacy at the University of North Carolina at Chapel Hill and the Director of the UNC Hospitals Ambulatory Care PGY2 Residency Program. She is the Assistant Medical Director of the UNC Internal Medicine Clinic and their Director of the Enhanced Care Programs where she providesg oversight of the mid-level provider clinics including anticoagulation, diabetes and chronic pain. In addition, she is Director of the UNC Eshelman School of Pharmacy PY4 Ambulatory Care Clinical Scholars Program at UNC Hospitals. She teaches medical residents, pharmacy residents and students within the disease management clinics, focusing on anticoagulation and Medicare Wellness.
Her professional interests include billing for pharmacy services, chronic disease management, quality improvement, health literacy and more recently is working on medication reconciliation efforts in transitions of care.

Amy Stump,
PharmD, BCPS, Clinical Pharmacy Specialist, Ambulatory Care,
Indiana University Health
Amy L. Stump, PharmD, BCPS, is a graduate of the University of Nebraska Medical Center College of Pharmacy and completed a Primary Care Specialty Residency with Mission Hospitals/MAHEC in Asheville, NC. She has worked as clinical faculty with the Auburn University Harrison School of Pharmacy and the University of Wyoming School of Pharmacy. She has practiced in a variety of settings including rural health clinics, Family Medicine Residency Training programs and private physician office practices. Currently, she is a Clinical Pharmacy Specialist in Ambulatory Care with the Indiana University Methodist Family Medicine Residency Training Program in Indianapolis. Her main practice interests include chronic disease management, wellness/disease prevention and creating sustainable pharmacist services.

- Compare other methods of offering financial value to create a sustainable program through cost-avoidance, pay-for-quality initiatives (PCMH, ACO), state Medicaid programs that have pharmacist provider status.
- Discuss financial incentives available by payors for established clinics with collaborative drug therapy management (CDTM).
- Explain the key elements necessary when conducting a needs assessment to include: initial scope of services, role of students/residents, and interdisciplinary patient centered care when developing a business proposal.
