Berook Addisu,
PharmD, BCPS, Associate Chief,
William S. Middleton Memorial Veterans Hospital
Berook Addisu, Pharm.D., BCPS, is the Associate Chief of Pharmacy Service at the William S. Middleton Memorial Veterans Hospital in Madison, Wisconsin. In this role, Dr. Addisu assists the Chief of Pharmacy with planning, directing, and coordinating comprehensive pharmacy services to veterans residing in the south-central Wisconsin and northwestern Illinois regions. He participates in many facility committees and workgroups including the Pharmacy Practice Model Initiative Committee. He is also involved in the training of residents and serves as a Clinical Instructor for University of Wisconsin-Madison (UW-Madison) School of Pharmacy students. His current practice interests include acute care management and clinical development.
Dr. Addisu earned his Pharm.D. at the UW-Madison. He completed a PGY-1 Pharmacy Practice Residency at the VA Tennessee Valley Healthcare System in Nashville and Murfreesboro, TN. He then completed a PGY-2 Health-System Pharmacy Administration Residency at his current facility. In addition to ASHP, he is also a member of the Pharmacy Society of Wisconsin and a board member of the UW-Madison Pharmacy Alumni Association. Dr. Addisu is a Board Certified Pharmacotherapy Specialist.

Mr. Calabrese is currently the Associate Chief Pharmacy Officer at The Cleveland Clinic in Cleveland, Ohio. He obtained his bachelors degree in pharmacy from the Philadelphia College of Pharmacy and Science and his Masters degree in Business Administration from Cleveland State University. In his role, Mr. Calabrese has oversight of clinical services and pharmacy operations for Cleveland Clinic main campus and the Cleveland Clinic Childrens Hospital for Rehabilitation which includes over 136 technicians and 100 pharmacist FTEs. He has severed in several leadership roles while at the Cleveland Clinic including Pharmacy Manger for Decentralized Services, Pharmacy Manager for the Childrens Hospital and Taussig Cancer Center, Assistant Director and Director of Pharmacy Services.
He completed a redesign of the medication distribution system including implementation of automated carousels resulting in a dramatic decrease in turnaround times for medications. He also implemented a career ladder for the technical staff, developed a work at home program for pharmacists and developed a new pharmacy management team.
Mr. Calabrese is active in the Ohio Society of Health System Pharmacists and is a past president of the organization. He is also active in the American Society of Health System Pharmacists where he serves as a delegate for Ohio. In 2009 Mr. Calabrese was awarded the Timothy D Moore Management award by the Ohio Society of Health System Pharmacists.

Lourdes Cuellar,

TIRR Memorial Hermann
Lourdes Cullar currently serves as Administrative Director of Pharmacy and Clinical Support Services at TIRR Memorial Hermann in the Texas Medical Center in Houston. She is responsible for the leadership and management of the Pharmacy, and administratively responsible for the clinical services departments at TIRR. She received both her B.Sc. Pharm and M.S. in Pharm Administration from the University of Houston.
She is a past-president of the Texas Society of Health-System Pharmacists, Gulf Coast Society of Health-System Pharmacists and past-president and chair of the TSHP Research & Education Foundation. She serves on the Texas Statewide Health Coordinating Council by appointment from Governor Rick Perry.
She is an adjunct Clinical Professor of Pharmacy Practice and Administration at the University of Houston College of Pharmacy and the University of the Incarnate Word Feik School of Pharmacy. She serves as a preceptor for students 4 of the 6 colleges of Pharmacy in Texas and for Memorial Hermann Health System combined MS/Residency program. She is also co-editor with Diane Ginsburg of the ASHP Preceptors Manual.
Her professional interests are in the areas of pharmacy practice management, mentoring pharmacy students and residents, leadership development, health care policy, alleviation of health disparities, and health literacy & cultural compentence. She is also recognized as a Fellow of the American Society of Health-System Pharmacists and of the ASHP/Cardinal Health/University of Boston Leadership Institute.

Trish Hessling,
PharmD, Clinical Pharmacist,

Trish Hessling, PharmD is currently a Clinical Pharmacist at the William S. Middleton Veterans Hospital in Madison, Wisconsin. She received her Doctor of Pharmacy degree from the University of Wisconsin in 2005 and completed her Primary Care Specialty Residency at the Veterans Hospital in Madison in 2006. Her practice experience has been primarily in infectious disease, home infusion services, medication management and anticoagulation but she also works as a night pharmacist in the inpatient pharmacy.

Susan Joyce,

Susan Joyce is a clinical pharmacist at the William S. Middleton VA in Madison, WI. She received a degree in Biology at the University of Illinois and a Doctorate in Pharmacy at the University of Wisconsin. She has received certification as a Board Certified Pharmacotherapy Specialist. Currently she works as the facility's tobacco cessation coordinator and assists in other pharmacy run outpatient clinics.

Thomas Kirschling,

THOMAS E. KIRSCHLING, PharmD, MS, MSL, is Director of Acute Care Pharmacy and responsible for pharmacy services throughout Denver Healths 525-bed Academic Level 1 Trauma Center, assisting Colorado's primary safety net institution by providing access to the highest quality pharmaceutical care regardless of patients ability to pay. Prior to joining Denver Health, Tom was Manager of Pharmacy Operations at University of Pittsburgh Medical Center (UPMC) and Adjunct Instructor for the University of Pittsburgh, School of Pharmacy. He received his Doctor of Pharmacy and Masters of Science from the University of Wisconsin, Master of Studies in Law from the University of Pittsburgh, Lean Black Belt from Denver Healths Lean Academy, and Six Sigma Green Belt from Motorola University. Tom is dedicated to the advancement of the profession through ASHP. He is Chair of the Section of Pharmacy Practice Managers Educational Steering Committee, and he has served as Chair of ASHPs Council on Pharmacy Management and as Chair at many Midyear Clinical Meeting programs.

Scott Knoer,
MS, PharmD, Chief Pharmacy Officer,
Cleveland Clinic
Dr. Knoer is the Chief Pharmacy Officer at the Cleveland Clinic Foundation. He is an international leader in health-system pharmacy administration. He has chaired the ASHP Section of Pharmacy Practice Managers (SPPM) Executive Committee and is a member of the University Health-System Consortium (UHC) Pharmacy Executive Committee. He has served in the ASHP House of Delegates, as Chair of the SPPM Advisory Group on Leadership Development, and as a member of the ASHP Commission on Affiliate Relations. Dr Knoer has published in the International Pharmacy Journal, the American Journal of Health-System Pharmacy and the Journal of Managed Care Pharmacy. He is a frequent invited lecturer on a broad variety of pharmacy topics including the pharmacy enterprise, the pharmacy practice model, pharmacy operations and automation, process improvement, leadership development, change management and disaster preparedness. Dr. Knoer is the co-creator of the ASHP Managers Bootcamp, a program designed to teach leadership and management skills to pharmacists promoted to leadership positions who lack formal training. Knoer also created combined M.S. / Administrative Residency programs at the University of Minnesota Medical Center and the Cleveland Clinic to train tomorrows pharmacy leaders. The Cleveland Clinic pharmacy enterprise consists of nine hospitals, 17 family health centers, five ambulatory surgery centers and 15 ambulatory pharmacies with services in Ohio, Florida, Nevada, Toronto (Ontario, Canada) and Abu Dhabi, UAE.

Heather Kokko,
PharmD, MBA, Director of Pharmacy Services,
Medical University of South Carolina
Dr. Heather Kokko is the Director of Pharmacy at the Medical University of South Carolina in Charleston. She also serves as the Clinical Associate Dean of Medical Center Affairs for the South Carolina College of Pharmacy.
Dr. Kokko earned her Doctorate of Pharmacy from the Medical University of South Carolina and a Masters in Business Administration from The Citadel Graduate College. She has also completed a 24-month pharmacy practice management residency at MUSC.
Dr. Kokkos main area of interest is in promoting the profession through expansion of clinical pharmacy services in health systems. She is committed to education of future pharmacy leaders through her roles as the Director of the Dual PharmD/MBA program at the South Carolina College of Pharmacy and the Graduate Pharmacy Educator for residency programs at MUSC Medical Center. She works closely with other healthcare leaders to promote medication safety through robust pharmacy services.

Jeffrey Little,
PharmD, MPH, Assistant Director of Pharmacy,
Children's Mercy Hospital
Jeff received his Doctor of Pharmacy from the University of Kansas School of Pharmacy. After graduation, he completed a two year post-graduate residency in health systems pharmacy administration at the University of Pittsburgh Medical Center in conjunction with a Master of Public Health from the University of Pittsburgh Graduate School of Public Health. Since completion of his residency, Jeff served as first as Pharmacy Operations Manager then as Assistant Director of Pharmacy at Childrens Mercy Hospitals and Clinics. His current responsibilities include oversight of all acute care pharmacy services at both inpatient Childrens Mercy campuses as well as pharmacy purchasing and finance for the health system. Jeff holds an adjunct faculty appointment at the University of Kansas School of Pharmacy and is the current treasurer for the Kansas Council of Health-Systems Pharmacists.

Christopher Lowe, PharmD, BCPS is the Clinical Pharmacy Manager at IU Health - University Hospital. Having trained at the University of Pittsburgh and The Johns Hopkins Hospital, Dr. Lowe also maintains a clinical practice in the area of hematologic malignancies and stem cell transplant.

Eric Maroyka,
PharmD, BCPS, Director, Department of Pharmacy,
Fort Belvoir Community Hospital
Lt. Col. Eric Maroyka earned his Bachelor of Science and Doctoral degrees in Pharmacy from Rutgers University. He completed an ASHP-accredited residency in Pharmacy Practice at Walter Reed Army Medical Center and is a Board Certified Pharmacotherapy Specialist (BCPS).
Eric earned his commission as a Second Lieutenant in the Army Medical Service Corps and his military education and training includes: the Army Airborne School, Combat Casualty Care Course, Medical Management of Chemical and Biological Casualties Course, and the Command and General Staff Officers Course.
Lt. Col. Maroyka has held a variety of leadership positions in his 20 year career as an Army Pharmacist to include assignments at: Landstuhl Regional Medical Center, Germany; Martin Army Community Hospital, Fort Benning, GA; Kimbrough Ambulatory Care Center, Fort Meade, MD; Reynolds Army Community Hospital, Fort Sill, OK; U.S. Army Hospital, Heidelberg, Germany; and Multi-National Corps-Iraq. Eric currently serves as the Director, Department of Pharmacy for the Fort Belvoir Community Hospital, Fort Belvoir, VA.
Some of his military awards and decorations include the Bronze Star Medal; Meritorious Service Medal; the Iraqi Campaign Medal, the Expert Field Medical Badge; and the Order of Military Medical Merit.
Eric completed training as an Officer-in-Residence with ASHP and is a graduate of the Pharmacy Leadership Institute.
Eric resides in Fairfax, Virginia with his wife Monica and their two daughters, Juliann and Allyson.

Richard Ogden,
PharmD, Manager, Pharmacy Critical Care Services,
Children's Mercy Hospital and Clinics
Richard Ogden Jr. received his PharmD from the University of Missouri-Kansas city in 2007. He completed a PGY-1 Residency at Children's Mercy Hospitals and Clinics in Kansas City, Missouri in 2008 and began his career in the Emergency Department at Children's Mercy. In 2011, Richard became the Pharmacy Manager for Critical Care Services at Children's Mercy which encompasses the Intensive Care Nursery, Pediatric Intensive Care Unit, and the Emergency Department. Richard received his BCPS in 2009.

Katherine Palmer,
PharmD, Pharmacy Program Coordinator,
Cedars-Sinai Medical Center
Katherine Palmer is currently a Pharmacy Program Coordinator at Cedars-Sinai Medical Center in Los Angeles, California. Her position includes coordination of patient-centered initiatives related to transitions of care and sterile compounding area projects.
She received her pharmacy degree from the University of Illinois at Chicago College of Pharmacy. She completed a PGY-1 Pharmacy Practice Residency at Oregon Health and Science University in Portland, Oregon, and a second year Pharmacy Practice Management Residency at Cedars-Sinai Medical Center in Los Angeles, California.
Over the past few years, she has served on the New Practitioners Forum as an Advisory Group Member, Advisory group Chair, and Vice-Chair of the New Practitioners Forum Executive Committee. She is currently Chair of the New Practitioners Forum Executive Committee.
Her professional interests include leadership development, medication safety, and working with pharmacy residents.

Brian Peters,
PharmD, MS, Director of Pharmacy,
Community Health Network
Brian has been the Director of Pharmacy for a 410-bed community hospital for the last six years. He has interest in pharmacy operations, specifically related to optimizing pharmacy staffing mix and leveraging automation to maximize staff efficiency. Brian has also been heavily involved in numerous technology implementations, including implementation of three different barcode medication administration systems.
Brian's training includes a Doctor of Pharmacy from Purdue University and a 2-year residency/Masters program in pharmacy administration from The Ohio State University. He is also a 2006 graduate of the Pharmacy Leadership Institute through Boston University.
Brian has been active in local and state pharmacy societies over the last 10 years, including serving as president of the Health-System Pharmacy academy of the Indiana Pharmacists Alliance in 2008. He has been a member of ASHP since 1995.

Has worked in Home IV therapy, Long Term care and retail pharmacy as well. Has served as President of the Maine Society of Hospital Pharmacists. Has also served as Board Chairman for Androscoggin Home Care and Hospice.

Elaine received her Doctor of Pharmacy at the University of Georgia. She completed her PGY1-PGY2 combined residency at the University of Pittsburgh Medical Center (UPMC) in Health Systems Pharmacy Administration along with a Master of Science from the University of Pittsburgh School of Pharmacy in Health System Pharmacy Administration. Elaine currently is the Pharmacy Manager of Clinical Services at West Penn Allegheny Health System in Pittsburgh, Pennsylvania where her primary focus is implementing initiatives related to pharmacy practice model expansion and transitions of care programs. At the time of presentation submission, she was actively involved in leading the pharmaceutical waste team and RCRA program implementation at UPMC. Elaines professional interests include practice model expansion, regulatory affairs, and resident and student development.

Mark Sullivan,

Mark is currently at Vanderbilt University Medical Center in Nashville Tennessee. He is the Director for VUH Pharmacy Operations for the Vanderbilt University Hospital. He also has been either responsible for or assisted with implementation of a number of medication related systems, including automated medication vending systems, Smart Pump medication infusion systems, the computerized prescriber order entry system and the bedside bar coding system. He has served as an invited member of the USP Safe Medication Use committee, and on the ASHP Practice Managers section advisory group on communications, most recently as vice chair. He holds faculty appointments to the University of Tennessee, Belmont University and Lipscomb University Colleges of Pharmacy and served on the Admissions Committee for the inaugural class of the Lipscomb College of Pharmacy.

Rebecca Taylor PharmD, MBA, BCPS graduated from the University of Pittsburgh School of Pharmacy in 2003. She then completed a PGY1 pharmacy practice residency at Aurora Health Care in Milwaukee, WI. She currently serves as the Pharmacy Education Manager at Cleveland Clinic and is Assistant Professor of Pharmacy Practice at Northeast Ohio Medical University (NEOMED).
