Perry Allen,

Perry Allen, PharmD is the Pharmacy Manager at Brunswick Novant Medical Center in Bolivia, NC. In this role, he oversees all aspects of pharmacy services, precepts students and leads pharmacy tranformation. Dr. Allen received his Bachelor of Science in Pharmacy and Doctor of Pharmacy with Distinction from The University of North Carolina at Chapel Hill.

Peggy Bickham, PharmD is Assistant Director of Pharmacy, Specialty and Support Services at the University of Illinois Hospital and Health Sciences Center, and Clinical Assistant Professor at the College of Pharmacy. Current responsibilities include management of the operating room pharmacy and oversight of medication use in the imaging and procedure areas. Dr. Bickham also serves as preceptor for 4th year pharmacy students on hospital practice rotation.

Alina Bulgar,

Alina Bulgar, PharmD, PhD, is the Coordinator, Medication Regulatory and Accreditation Services in the Department of Pharmacy at Cleveland Clinic. Alina Bulgar has obtained her PharmD from Victor Babes University of Medicine and Pharmacy in Romania. Then, she has completed her PhD degree in bioanalytical and clinical chemistry at Cleveland State University studying the involvement of the base excision repair (BER) pathway in the response to nucleotide analogs (e.g. fludarabine) induced DNA damage. Alina has then successfully continued her research as a postdoctoral scholar at the Case Western Reserve University School of Medicine, where she studied a potential new clinical targeted therapy. Her work resulted in more than 16 scientific contributions, including 4 articles published in prestigious peer reviewed journals. Dr. Bulgar has also co-authored a book chapter on classical alkylating agents in Cancer Chemotherapy and Biotherapy: Principles and Practice, 5e, LWW, 2010. Since 2010, Alina Bulgar has joined the Department of Pharmacy at Cleveland Clinic and her work focused primarily on analysis of the medication management system to maintain compliance with regulatory and accreditation requirements and to mitigate potential system vulnerabilities.

Nicole Clark,

Nicole J. Clark PharmD, BCPS graduated with her Doctor of Pharmacy degree from Massachusetts College of Pharmacy and Health Sciences in 2004 and became a Board Certified Pharmacotherapy Specialist in 2008. Dr. Clark is currently the System Pharmacy Clinical Manager and Residency Program Director at Hallmark Health System (Melrose Wakefield Hospital and Lawrence Memorial Hospital). She transitioned to the clinical manager role after working at Hallmark as the Unit Based Pharmacist Coordinator specializing in critical care. As clinical manager Dr. Clark is responsible for directing development, implementation and maintenance of clinical pharmacy services. She also facilitates the development and implementation of treatment guidelines, protocols, cost-savings initiatives, formulary changes and critical pathways. Dr. Clark currently precepts pharmacy students and is very involved in community outreach activities. She is the Chair of the Massachusetts Society of Health System Pharmacists Annual Meeting Committee, a 2011 American Society of Health System Pharmacists Foundation Pharmacy Leadership Academy Participant, is currently enrolled in the Masters and Health Administration Program at Simmons College and is a peer reviewer for American Journal of Health System Pharmacists.

Brenda Egan,

Brenda A. Egan, PharmD., BCPS is a Clinical Manager for Cardinal Health Solutions and is based at Bristol Hospital, CT. She has worked in various capacities with experience in three major areas of pharmacy that include Hospital, Community Care and Long Term Care. She received her Bachelors of Science in pharmacy from the University of Connecticut and later her Doctorate degree in pharmacy from the University of Kansas. Brenda is an adjunct instructor for pharmacy practice for both the University of Connecticut and University of Saint Joseph schools of pharmacy. She is also the secretary of Bristol Hospital's Pharmacy and Therapeutics committee, year round educator for clinical nursing competency fairs, is the pharmacy resource for Bristol Hospital's Hospice Interdisciplinary Group and was recently awarded the status of Board Certified Pharmacotherapy Specialist.

Timothy Ekola,

Tim graduated from the Ferris State University with his B.S. degree in 1994 and received his Doctor of Pharmacy at Shenandoah University. Throughout his career, he has worked as a clinical pharmacist on Med-Surg Wards, Operating Room Pharmacist, and Drug Information Specialist for Pharmacia (Pfizer). In 2006 he became Director of Pharmacy for Borgess-Lee Memorial Hospital in Dowagiac, MI. His military career spans over 24 years of medical service including multiple humanitarian deployments. His most recent deployment in support the Operation Iraq Freedom earned him several awards for excellence. He retired from the Navy Reserve in 2008 after 24 years of service. He is currently the Medication Safety Pharmacist at Landstuhl Regional Medical Center in Landstuhl, Germany. In his current role, he provides a leadership role for formulating policy and instituting models that promote medication safety in all aspects of the medication management process. He is responsible for coordinating all activities related to medication management and medication safety provided by department of pharmacy. These activities involve reporting, system and process evaluation, formulation of recommendations and initiatives, coordination of intervention strategies, and monitoring of activities related to medication safety.

Lynn Eschenbacher,

Lynn Eschenbacher, Pharm.D., MBA has a strong interest in process improvement and medication safety. After obtaining a BS in biology from Indiana University in Bloomington, IN she attended the University of Texas at Austin to obtain her PharmD. She then completed a pharmacy practice residency at Parkland Health & Hospitals in Dallas, Texas. Lynn worked at Duke University Hospital as a clinical pharmacist, clinical coordinator and medication safety officer. Additionally, she completed her MBA at the Fuqua School of Business at Duke University. She is currently the Assistant Director of Clinical Services at WakeMed Health & Hospitals in Raleigh, North Carolina and the PGY-1 Residency Director. Lynn has received the Circle of Excellence award at WakeMed for Market Development and Quality Outcomes. She is also the ASHP Delegate for the Acute Care Forum in North Carolina, a past-chair of the ASHP Section Advisory Group for Medication Safety, and the chair for the ASHP Section for Inpatient Care Practitioners.

Julia Gannon,

Julia Gannon is the Chief, Pharmacy Service for the United States Army MEDDAC at the Heidelberg Army Health Center (HMEDDAC), Germany. She was formerly the HMEDDAC Assistant Chief, Pharmacy since August 2001. As the Pharmacy director, Ms. Gannon is responsible for planning, coordinating and executing all areas of clinical and distributive pharmacy to three geographically separated pharmacies and the clinical program and ambulatory pharmacy at the Heidelberg Army Health Center. Prior to her assignment in Heidelberg, Ms. Gannon was Chief of Pharmacy in Fort Monmouth. Other assignments include Outpatient Chief of Pharmacy, Fort Eustis, and the 97th General Frankfurt Army Regional Medical Center in Frankfurt, Germany. Her overall experience encompasses more than 25 years of professional pharmacy skills in the areas of hospital and ambulatory pharmacy to include quality control, performance improvement, patient safety, personnel management, training and patient relations. Ms. Gannon represents Army civilian pharmacists as the Army Civilian Counsel Chairperson. Ms. Gannon is a graduate of the Long Island University, AMS College of Pharmacy and Health Sciences. She is a certified Lean Six Sigma Black Belt and a credentialed clinical pharmacist for Anticoagulation. She attended the University of Pennsylvania Leadership Pharmacy Development Institute at the Wharton School of Business and is currently enrolled in the ASHP Pharmacy Leadership Academy and affiliated Master's in Health Administration program with Simmons College.

Joseph Hooley,

Joseph Hooley earned his Bachelor's Degree in Pharmaceutical Sciences from the State University of New York (SUNY) at Buffalo in 2006 and is currently enrolled in the Cleveland State University Master of Business Administration (MBA) program, with a concentration in Health Care Administration. Mr. Hooley is currently the Adverse Drug Event Coordinator at Cleveland Clinic, providing medication safety support to all hospitals within the Cleveland Clinic Health System (including adverse drug event review and analysis, medication error prevention, and quality improvement initiatives). Mr. Hooley has five years of experience working in the medication safety field, as well as prior experience as a pharmacy technician in hospital, retail, and long-term care settings. Mr. Hooley is also faculty for the Cleveland Clinic School of Pharmacy Technology, providing medication safety education to pharmacy technician students.

Molly Leber,

Molly Billstein Leber, PharmD, BCPS is currently the Clinical Pharmacy Coordinator at Yale New Haven Hospital (YNHH) in New Haven, Connecticut. She obtained her PharmD at the University of Montana and completed a Pharmacy Practice Residency at YNHH. Molly has worked at YNHH as the Medical ICU Clinical Pharmacist and Anticoagulation Clinical Specialist. Currently she is the Clinical Advisor to the Pharmacy and Therapeutics Committee and is also the Interim-Clinical Coordinator of the Yale New Haven Health System Formulary Integration Committee (FIC). FIC was created to standardize the independent medication formularies into one Health System formulary to enhance medication safety, increase cost savings and promote evidenced based practices.

Doctorate: University of Kansas, 4.0 GPA. 5 years system level experience working with UH Quality Institute, Informatics and IT&S to facilitate initiatives on behalf of leadership. Mary Rebo,

Elizabeth Rebo, PharmD is the medication safety manager for Novant Health. In this role, Dr. Rebo is responsible for ensuring safe medication use practices across Novant using multidisciplinary collaboration. She is also responsible for continuous regulatory readiness across the corporation. Dr. Rebo is a Clinical Assistant Professor of Pharmacy at Wingate University, in addition to precepting PGY1 pharmacy residents from Forsyth Medical Center. Prior to this role, Elizabeth served as the medication safety coordinator of the Triad Region at Novant Health with similar responsibilities. Past practices also involve working as the investigational drug studies clinical specialist at Forsyth Medical Center and a clinical staff pharmacist in critical care at Wake Forest University Baptist Medical Center.
Dr. Rebo received her Bachelor of Science in Biological Sciences from Clemson University in 2000 and her Doctor of Pharmacy from the University of South Carolina in 2004.

Deborah Saine,
MS, RPh, Medication Safety Manager,
Winchester Medical Center
Deb Saine is the Medication Safety Manager for Winchester Medical Center - Valley Health System, in Winchester, Virginia. She brings over 20 years of medication safety expertise to her current position, where she initiates and oversees safety and quality activities. Deb also introduces medication safety practice to students and residents through experiential rotations and teaching at the Shenandoah University Bernard J. Dunn School of Pharmacy.
Ms. Saine earned her B.S. in Pharmacy from the University of Toledo, and an M.S. in Management from Antioch University. She has extensive clinical and leadership experience in community hospital and academic medical center settings.
Deb is an enthusiastic champion of medication safety, evident in her numerous publications and presentations in the U.S. and internationally. She is editor of the upcoming Medication Safety Officers Handbook, and of ASHPs Safety and Quality Pearls 2.
Deb has served the pharmacy profession in State and National leadership roles, and assists ASHP in advising the Joint Commission on medication safety issues. She is a Fellow of ASHP, and was presented with the ASHP Distinguished Service Award in 2011.

Aki Singam,

Dr. Singam began her career as the pharmacist in-charge of night pharmacy operations, and translated her experience into performance improvement and quality assurance, both in the private and public sectors. She created the first protocol for improving the outcomes from open-heart surgery and was selected for a leadership role. As the Medication Safety Manager at Walter Reed Army Hospital she was awarded the Commanders Award for Civilian Service. The citation states that Dr. Singams exceptional commitment to coordinate medication safety initiatives and integrate best practices during transition has set her apart as a distinguished pharmacist during BRAC integration. Her dedication was critical to the success of several highly visible and successful programs across Walter Reed, which influenced the development of integrated pharmacy policy and procedure across the national capital area.

Jennifer Sterner-Allison is currently the Clinical Pharmacy Manager at the Scottish Rite campus of Children's Healthcare of Atlanta. Current responsibilities include oversight of medication use policies/guidelines, provision of drug information, and expansion of clinical pharmacy services. She is actively involved in medication safety initiatives, electronic medical record implementation and maintenance, and training pharmacy students and residents.

Jules M. Trahan, PharmD, RPh, is currently the Medication Safety Fellow in a two-year post-doctoral fellowship program at Saint Vincent Hospital in Worcester, MA. He is also an adjunct member of the faculty at Massachusetts College of Pharmacy and Health Sciences - Worcester. Jules received his Doctor of Pharmacy degree from Northeastern University in May 2011. While at Northeastern, he gained clinical experience at institutions including Boston Childrens Hospital, Spaulding Rehabilitation Hospital, CVS Pharmacy, Boston Medical Center, Brigham and Womens Hospital, Inman Pharmacy, Biogen-Idec, Dana Farber Cancer Institute, and Dr. Solomon Carter Fuller Mental Health Center. Jules is a member of ASHP and APhA. He currently resides in Boston, MA.
