Description
At the 2007 ASHP Midyear Clinical Meeting in Las Vegas, Darryl S. Rich of
the Joint Commission, in response to a question, indicated that heparin
used for the prophylaxis of DVT was included in scope of the new National
Patient Safety Goal Requirement 3E on anticoagulation management. Since
the meeting, Dr. Rich, has checked with The Joint Commission headquarters,
and found out that prophylaxis using subcutaneous heparin (unfractionated
and LMW) will not be included in the scope of the goal requirement. He
apologizes for the misinformation. Further information on the requirements
of the goal will be released on the Joint Commission website in its 2008
NPSG FAQ in the first quarter of 2008. Any information regarding the
requirements should not be taken as final until this document is released.
LEARNING OBJECTIVES
- List the three most problematic medication management standards and the five most problematic medication-related National Patient Safety Goals requirements in terms of non-compliance by organizations surveyed in 2007, and the most common reason why each was scored non-compliant.
- Cite the medication management requirements for 2007 and 2008, including MM.4.10, pharmacist review of orders in the emergency room and radiology.
- Describe the requirements for compliance with the new 2008 NPSG for anticoagulation management.
- Describe an approach to developing an anticoagulation management program that meets the requirements of NPSG 3E.
- Describe how a data flow map can be used to analyze risk and improve your medication management system to meet the requirements of standard MM.8.10.
