Learning Objectives:
Define palliative care and identify the evolving role of palliative care throughout the continuum of care. Identify and explain the role of pharmacists within an interdisciplinary team in optimizing palliative care. Explain how to develop a comprehensive plan of care based on the interdisciplinary assessment of the patient and family. Recommend appropriate modalities (pharmacologic and non-pharmacologic) in the management of common physical and emotional symptoms and medication-related side effects. Identify challenges related to caring for patients with advanced illness across different venues and coordination of care. Explain how to develop competencies for pharmacy staff in pain and palliative care.
