Leslie is currently the Program Director for Computerized Provider Order Entry Systems at Duke and has completed implementation of the McKesson Horizon Expert Orders system in adults and pediatrics. This system is rich in decision support. She has a leadership role in the integration of Clinical information systems at Duke. Her team of 18 analysts has responsibility for oversight of the McKesson®, Varian® and Wellsoft® order entry systems, medication and allergy integration, and VARIAN Med and Radiation oncology systems.
She has practiced for most of her professional career at Duke University Health System, serving in a variety of capacities in the Department of Pharmacy and most recently in Hospital Information Systems. Her past responsibilities have included oversight of all areas of Pharmacy. Leslie assumed a role which included oversight of all Pharmacy Information systems, automation and the departmental liaison to information systems throughout Duke in 1993. In this capacity she implemented numerous pharmacy systems including Cerner, Pharmnet, the McKesson Rxobot and HMM systems, and Pyxis as well as IT oversight of the departmental network, Web integration, outpatient, inventory and custom database systems.
Ms Mackowiak earned a baccalaureate degree from the University of Illinois and a master degree in pharmacy at the University of North Carolina at Chapel Hill. She has lectured on many aspects of information systems. She has represented pharmacy in many capacities both within the state and nationally. She currently represents NCAP on the NCHICA Standing Advisory Committee.

Description
Electronic clinical decision support (CDS) requires multiple ‘parts’ and ‘pieces’ to work effectively within computer systems – ‘parts’ include processes for display and notification to providers, frameworks for organizing information, hyperlinks, data gathering used in rule evaluation, rules engines, data standards, surveillance, output measures, and outcome measures. As we work to create active alerting rules that can be shared among hospitals and within systems, it is crucial for pharmacists to understand the ‘parts’ and ‘pieces’ and how they work together to best support medication use. Many vendor systems supply application level tools which support some of these parts and processes and allow pharmacists to configure rules within these frameworks, and understanding these as ‘parts’ allows pharmacists to better manage CDS locally and share information. This session will address (1) active work being done by some pharmacists in this arena as well as (2) data standards and the use of RX Norm to identify medications across CDS systems.
- Describe how to use RxNorm within eMeasure (Clinical Quality Measure) rules and value sets and within ePrescribing.
- Describe the parts necessary to support reactive CDS alerts.
- Explain how to reconcile RxNorm reported patient medications and medication allergies to enterprise interface terminologies.
- List specific issues with sharing CDS alerts today and explain how to design CDS across all of the parts.
