Description
Recipient of the ASHP Foundation’s Award for Excellence in Medication-Use Safety.
The elderly, especially those with cognitive difficulties, are at greater risk for medication safety adverse events after hospital discharge. The VA Boston Healthcare System (VABHS) developed the Pharmacological Intervention in Late Life (PILL) Service as a safety net program whereby pharmacists provide both telephone-based medication management as well as in-person clinic-based visits alongside a physician geriatric specialist. Use of the PILL Service has resulted in fewer emergency department visits, hospital readmissions and deaths, and has reduced the health system’s costs. In this session, VABHS pharmacy staff will detail how the program was implemented and share lessons learned; in addition, they will describe how such programs can be implemented in your setting.

  • Describe a pharmacist-led process to address medication issues after discharge, as demonstrated by the PILL Service.
  • Identify key principles involved in post-discharge medication review and reconciliation.
  • Identify medication safety risks associated with cognitive impairment, and the heightened risk during transition home from the hospital, for older adults.
  • Recognize how to apply concepts presented in this session, to enhance medication safety and reconciliation for high-risk patients at your practice site.
