Mary Andrawis, PharmD, MPH, Field Co-Director, Medication Safety, Center for Medicare & Medicaid Innovation (CMMI)/Centers for Medicare & Medicaid Services (CMS)
Mary A. Andrawis, Pharm.D., M.P.H., serves on the core Partnership for Patients team within the Center for Medicare and Medicaid Innovation. The aims of the HHS-wide initiative are to reduce preventable harm by 40% and reduce readmissions by 20% by the year 2013.
Dr. Andrawis trained in patient safety through Johns Hopkins Hospitals Pharmacy Administration residency in Baltimore, Maryland, a two-year program combining intense clinical activities with administrative training in pharmacy management, quality improvement, patient safety, and emergency preparedness. She then served as Director of Clinical Guidelines and Quality Improvement at the American Society of Health-System Pharmacists, where she oversaw the development of ASHP therapeutic guidance documents and provided guidance to ASHP members and external stakeholder organizations to promote the safe and effective use of medications.
Dr. Andrawis is a graduate of South Dakota State University College of Pharmacy and the Johns Hopkins Bloomberg School of Public Health. She was born and raised in Brookings, South Dakota, where she continued into the College of Pharmacy and Honors College, graduating in 2006 with a Doctor of Pharmacy with Honors College Distinction and a minor in Spanish. She earned a Masters of Public Health from the Johns Hopkins Bloomberg School of Public Health in June of 2008 with an emphasis in Health Policy.
Janet Carmichael, PharmD, BCPS, VISN 21 Pharmacy Executive, VA Sierra Pacific Network (VISN 21)
Jannet M. Carmichael, Pharm.D., FCCP, FAPhA, BCPS is the VISN 21 Pharmacy Executive for the VA Sierra Pacific Network covering Northern Nevada, Northern California and Hawaii. Dr. Carmichael coordinates a Health Informatics, Pharmacoeconomic and Pharmacoepidemiology program using a Clinical Data Warehouse, PBM performance metrics for Veteran Integrated Service Network (VISN 21). Her clinical practice includes primary care and outcomes studies. Jan is Pharmacy Residency Program Director for a PGY2 program in Managed Care Systems & Pharmacoeconomics.
Jan is a Past-President of the American Society of Health-system Pharmacists, past member of the Nevada Board of Pharmacy, Past-President of the Nevada Pharmacists Association and of the Nevada Pharmacy Alliance. She participates as a member of numerous professional boards and committees. She is past-Chair of the Board of Pharmaceutical Specialties, has Chaired the BCPS Pharmacotherapy Council and is an ACPE Practitioner Reviewer. Jan recently served as a member of the National Quality Forum (NQF) Medication Management Sterring Committee and is a current member of the CMS Medication Measures Technical Expert Panel. Jan has authored hundreds of professional papers, book chapters and research projects. She is a frequent speaker on progressive pharmacy practice throughout the country.
Prior to coming to Reno, Dr. Carmichael taught at the University of Iowa, College of Pharmacy and the University of North Carolina, College of Pharmacy. Dr. Carmichael is a graduate of the University of Iowa College of Pharmacy and the University of the Pacific College of Pharmacy Pharm.D. program. She is a Board Certified Pharamcotherapy Specialist, a Fellow of the American College of Clinical Pharmacy and a Fellow of the American Pharmacist Association.

Shekhar Mehta, PharmD, MS, Director, Clinical Guidelines and Quality Improvement, American Society of Health-System Pharmacists
Shekhar Mehta, Pharm.D., M.S., is Director of Clinical Guidelines and Quality Improvement at the American Society of Health-System Pharmacists (ASHP), in Bethesda, Maryland. He earned his Master of Science in Biostatistics from the University of Pittsburgh School of Public Health in August of 2006, and Pharm.D. from the University of Maryland School of Pharmacy in 2010. While attending the University of Maryland he concurrently worked in the Biometrics and Data Management Department at Boehringer-Ingelheim Pharmaceuticals for 3 years helping develop clinical trial reports for submission to the FDA. Following the completion of his Pharm.D., Dr. Mehta mastered clinical skills and served the leadership role of being one of the first residents of an emerging PGY1 Pharmacy Practice Residency Program at Frederick Memorial Hospital, a small but diverse community hospital in Frederick, Maryland. Dr. Mehta joined the team at ASHP in the summer of 2011, where he coordinates and manages the development of ASHP therapeutic guidance documents in the compendium of Best Practices for Hospital and Health-System Pharmacy. He serves as an advocate on clinical quality improvement initiatives with various public and private sector organizations on behalf of ASHP.
Lam Nguyen, PharmD, Operations Manager, Oregon Health & Science University
Dr. Nguyen is an Operations Manager, Inpatient Pharmacy Services & Interim Pediatric Clinical Services at Oregon Health & Science University (OHSU). He graduated from the University of Colorado School of Pharmacy in 2009. Following graduation, he completed a PGY1 Pharmacy Practice Residency at University of Colorado Hospital and a PGY2 Health System Pharmacy Administration Residency at OHSU. Dr. Nguyen joined the OHSU management team in 2011 and has responsibilities over the inpatient hospital medication distribution process with emphasis in USP 795, USP 797, and technology implentation. He chairs the OHSU Pharmacy Quality Improvement committee and has a special interest in Lean process improvement and change management strategies. Clinical areas of interest: Pediatrics, oncology, and emergency medicine.
