Wayne Conrad, PharmD, Contract Surveyor, American Society of Health-System Pharmacists
Wayne F. Conrad, Pharm.D., FASHP is an Emeritus Professor at the University of Cincinnati and consultant for the American Society of Health System Pharmacists. In his consultant capacity he leads accreditation survey teams, develops training materials and conducts educational programs.
Dr. Conrad retired from the University of Cincinnati in September, 2011. At that time he was Professor and Chair of the Division of Pharmacy Practice and Administrative Sciences at the University of Cincinnati, James L. Winkle College of Pharmacy. He was also Director of Post-Graduate Training Programs and Residency Program Director for a Post Graduate Year One Residency Program in Community Pharmacy at the Winkle College of Pharmacy. He is a graduate of the State University of New York at Buffalo and the University of Kentucky colleges of pharmacy. He completed post-graduate residency training at Springfield City Hospital and is a fellow of the American Society of Health System Pharmacists. Dr. Conrad is a former faculty member of the University of Kentucky and State University of New York at Buffalo colleges of pharmacy.
Dr. Conrad has authored more than 50 publications and abstracts. His research interest include residency training methodologies, quality improvement and outcomes studies related to pharmacy services. Dr. Conrad was project director for the ASHP Model for Pharmacy Residency Learning Demonstration Project that was completed in 1996 and resulted in the Residency Learning System being used by most pharmacy residency programs today. He has taught courses in health care systems, institutional pharmacy practice, medication safety and quality improvement at three colleges of pharmacy.
Dr. Conrad has had more than 30 years experience as a director of pharmacy, residency program director and preceptor at three academic science centers. During his acreer he developed innovative programs regarding drug distribution and clinical roles for pharmacists. He developed residency programs at four institutions and trained more than one hundred pharmacy residents and graduate students.
Dr. Conrad has held several leadership positions in local, state and national pharmacy organizations, including president of the New York State Council of Health System Pharmacists in 1982 and two appointments to the ASHP Commission on Credentialing. He is a recipient of the Walter Frazier Award for outstanding contributions to the profession of pharmacy. Dr. Conrad has been a consultant for the ASHP Accreditation Services Division since 1988.
