James Hoffman,
PharmD, MS, Medication Outcomes and Safety Officer,
St. Jude Children's Research Hospital
James M. Hoffman, Pharm.D., M.S., BCPS, is Medication Outcomes and Safety Officer and Associate Member in Pharmaceutical Sciences at St. Jude Childrens Research Hospital in Memphis, Tennessee. At St. Jude, he leads medication use safety efforts across the hospital and conducts research focused on patient safety event detection, patient safety culture, and clinical decision support. Hoffman has been involved in a variety of patient safety and quality improvement efforts at St. Jude including development and implementation of the hospitals electronic event reporting system (EERS), patient safety culture assessment and improvement, and refinement of clinical decision support systems. He also has extensive experience analyzing various aspects of pharmaceutical use and policy, including leading an annual publication that projects medication expenditures.
Dr. Hoffman received both his Bachelor of Science and Doctor of Pharmacy degrees from the Philadelphia College of Pharmacy. In addition, he received a Masters of Science degree in pharmacy administration from the University of Wisconsin-Madison. He also completed a residency in pharmacy administration and a fellowship in outcomes research at the University of Wisconsin Hospital and Clinics. Dr. Hoffman is a Board Certified Pharmacotherapy Specialist. He is also an Associate Professor at the University of Tennessee Health Science Center College of Pharmacy.

Kerry Parsons,
PharmD, BCOP, Pediatric Oncology Pharmacist,
Children's of Alabama
Dr. Parsons obtained her BSPharm from Auburn University and her PharmD from Creighton University. She also has her board certification in oncology pharmacy.
Dr. Parsons is an experienced pediatric pharmacist that has focused the majority of her career in pediatric hematology-oncology. She currently works at Children's of Alabama in Birmingham as the lead oncology pharmacist and manages the oncology investigational drug service. She serves as an educator for the medical program and PGY2 oncology residency program at the University of Alabama in Birmingham.
In addition to her clinical practice, she's an active member of the Children's Oncology Group of which she's a steering member of the Adolescent and Young Adult Committee.

Christine Walko,
PharmD, BCOP, Assistant Professor,
University of North Carolina, School of Pharmacy
Chris Walko received her PharmD from Duquesne University in Pittsburgh, Pennsylvania, and worked as a research assistant at the University of Pittsburgh Cancer Institute. She completed her pharmacy practice residency at the Medical College of Virginia and her hematology and oncology specialty residency at the University of North Carolina at Chapel Hill. She also completed her academic fellowship in Hematology and Oncology Pharmacotherapy at the University of North Carolina. She joined the faculty at UNC in 2006 as a clinical assistant professor. She practices as a clinical pharmacist in the lung and melanoma clinics at the NC Cancer Hospital at UNC. Additionally, she is one of the pharmacokineticists for the UNC Lineberger Comprehensive Cancer Center Phase I program and the Director of the Clinical Pharmacology Lab in the NC Cancer Hospital. Her research program focuses on the utilization of pharmacokinetics and pharmacogenomics to individualize cancer therapy for patients.
