Abbott Northwestern Hospital - Allina Health System
Kristine K. Gullickson is the Director of Pharmacy at Abbott Northwestern Hospital, Allina Health, a 600 bed teaching and tertiary care referral center. She is responsible for leadership of a progressive pharmacy department with an integrated patient-centered practice model. The pharmacy department was a recent recipient of the ASHP Best Practices Award in 2011. At the Allina Health system level, Ms. Gullickson co-chairs the Allina Pharmacy Operations group with oversight for operations, safety and regulatory systems for 11 Allina hospitals. Her current and past professional association activities include: President of the Minnesota Society of Health System Pharmacists (MSHP), delegate to the ASHP House of Delegates, Member of the ASHP Council on Pharmacy Practice, Member of the Section of Pharmacy Practice Managers (SPPM) SAG on Quality and Compliance and Communications and Publications.

Shekhar Mehta, Pharm.D., M.S., is Director of Clinical Guidelines and Quality Improvement at the American Society of Health-System Pharmacists (ASHP), in Bethesda, Maryland. He earned his Master of Science in Biostatistics from the University of Pittsburgh School of Public Health in August of 2006, and Pharm.D. from the University of Maryland School of Pharmacy in 2010. While attending the University of Maryland he concurrently interned in the Biometrics and Data Management Department at Boehringer-Ingelheim Pharmaceuticals for 3 years helping develop clinical trial reports for submission to the FDA. Following the completion of his Pharm.D., Dr. Mehta mastered clinical skills and served the leadership role of being one of the first residents of an emerging PGY1 Pharmacy Practice Residency Program at Frederick Memorial Hospital, a small but diverse community hospital in Frederick, Maryland. Dr. Mehta joined the team at ASHP in the summer of 2011, where he coordinates and manages the development of ASHP therapeutic guidance documents in the compendium of Best Practices for Hospital and Health-System Pharmacy. He serves as an advocate on clinical quality improvement initiatives with various public and private sector organizations on behalf of ASHP.

Institutions and practitioners are faced with opportunities and challenges to improve the quality of healthcare. There are several national initiatives to improve quality and safety. This session will highlight the Center for Medicare and Medicaid Innovation - Partnership for Patients and ASHP members' efforts to support this national initiative.
- Describe the Center for Medicare and Medicaid Innovation - Partnership for Patients core programs focused on improving patient safety.
- Discuss and highlight opportunities for dissemination of best practices to improve outcomes.
- List examples of processes, procedures, and systematic approaches used by ASHP members who have had success decreasing adverse drug events and reducing hospital (re)admissions in their communities.
