I am a pharmacist with 30+ years of experience in hospital pharmacy practice, the majority of those years in leadership roles, including director of pharmacy and have been involved in myriad process improvement initiatives to improve the efficiency of the medication system, expand pharmacy services, design and open new pharmacies, to name a few. Currently, I am a consultant who helps hospitals improve their medication systems, and in this capacity, have worked with a great number of hospitals to successfully achieve this goal.

Compare and contrast the experiences and outcomes of two institutions that used Project RED (Re-engineered Discharge) to re-engineer the discharge process to incorporate pharmacists.
Describe the steps in the discharge process that are improved with pharmacist involvement.
Evaluate the challenges and potential solutions using hands-on techniques, including SWOT (Strengths, Weaknesses, Opportunities, and Threats) analysis and fishbone diagramming.
