As the Clinical Coordinator for the Pharmacy Department at Saint Luke’s I have been involved in the development and implementation of a number of new and unique clinical programs in our Baldrige Award winning hospital. With regard to medication-related falls and delirium I have served as a member of our organization’s multidisciplinary fall prevention team. Additionally, I have overseen several residency research projects in the area of sedative use and its relationship to patient falls. I have also served as supervisor to clinical pharmacists who work in the critical care areas and who have been involved in the development and implementation of a sedation protocol (designed to reduce or minimize delirium in ICU patients). At present, we are in the process of developing an algorithm to optimize the selection of sedative hypnotic agents in hospitalized patients.

This session will provide a keynote presentation on the Pharmacy Practice Model Initiative (PPMI) by a national thought leader. The session will continue with presentations from federal pharmacists, which will focus on the importance of federal pharmacy’s involvement in the new pharmacy practice model initiative and will showcase examples of patient-centered practice environments, discussing various projects that have been implemented in the federal sector resulting in improved patient outcomes. The session will also include a first of its kind federal pharmacy technician presentation that describes the outstanding roles and responsibilities that federal technicians have undertaken. Their contributions have improved the efficiencies of their departments and also resulted in freeing up the pharmacist for more direct patient care activities. The session will conclude with presentations from the Federal Pharmacy Chiefs discussing key issues and challenges facing federal pharmacy practice today.
