One of the major areas of patient care responsibilities for me has been the management of anticoagulation therapy, primarily in the inpatient setting. This has resulted in dozen of published manuscripts covering a wide number of related issues. I have also represented pharmacy on multi-disciplinary national events, and lectured on optimizing anticoagulation therapy. My practice setting has been highlighted in AJHP and by ACCP. Other related activities include serving as the chair of the editorial advisory board for The Annals of Pharmacotherapy panel on anticoagulation, and authored three chapters in the ASHP textbook on anticoagulation therapy edited by Michael Gulseth.

I have spoken on the area of anticoagulation at numerous national meetings. I have published several papers addressing the controversies in heparin monitoring.

Ann Wittkowsky,

Ann K. Wittkowsky, PharmD, CACP, FASHP, FCCP is Clinical Professor of Pharmacy at the University of Washington School of Pharmacy and Director of Anticoagulation Services at the University of Washington Medical Center. As a clinician and educator in the field of antithrombotic pharmacotherapy, she has contributed extensively to the care of patients and the education of health care providers regarding antithrombotic pharmacotherapy. She is the author of over 100 scientific papers, book chapters and abstracts, and co-editor of the textbook Managing Oral Anticoagulation Therapy: Clinical and Operational Guidelines. She has lectured widely throughout the United States and Canada, maintains an active clinical research program, and is board certified as an anticoagulation care provider. Dr. Wittkowsky serves as an editorial board member for Pharmacotherapy and the Journal of Thrombosis and Thrombolysis, as a reviewer for numerous medical and scientific journals, and as a member of the board of directors of The Anticoagulation Forum, a multidisciplinary organization of anticoagulation care providers. She is the director of the Northwest Anticoagulation Consortium, a preceptor for the ASHP Research and Education Foundation’s Antithrombotic Pharmacotherapy Traineeship, and a member of the American College of Chest Physician’s 8th Conference on Antithrombotic and Thrombolytic Therapy. She was recently elected to the Board of Regents of the American College of Clinical Pharmacy.

Workshop Fee: $210
Pre-registration is required; Registration must received on or before November 22.
Registration is limited to 60 participants. Lunch is on your own.
The National Patient Safety Goals on anticoagulation have generated new opportunities for pharmacists to develop professionally in this area. Designed for pharmacists who already have some experience in managing anticoagulation therapy, this advanced workshop employs a case-based approach to developing and implementing safe, effective management plans.
LEARNER OUTCOMES:
- Describe approaches to documenting and supporting continued activities related to managing anticoagulation therapy.
- Describe approaches to switching between, overlapping, holding, or reversing anticoagulants.
- Develop and communicate an anticoagulant management plan.
- Discuss approaches to establishing and revising protocols for anticoagulation therapy.
- Identify situations potentially predisposing patients to adverse anticoagulation related events and discuss approaches to initiating and monitoring anticoagulant therapy.
