Dr. Ghafoor graduated from the University of Nebraska Medical Center in 1991. She served as an oncology clinical coordinator from 1991-1994 at the University of Kansas Medical Center. In 1994, she received a 2-year fellowship award from the ASHP Research and Education Foundation for oncology research at the University of Wisconsin-Madison. Primary fellowship projects included development of HPLC assays for beta-lapachone and morphine metabolite identification. The fellowship involved working with nationally recognized leaders in cancer and pain management research. After completing the fellowship, Dr. Ghafoor accepted a faculty position with the University of Rhode Island College of Pharmacy. She conducted pharmacy education and research programs in pain management at Rhode Island Hospital in Providence. While on faculty, she developed the pharmacy practice residency program for the college of pharmacy and received ASHP-accreditation in 1999. In May 2000, Dr. Ghafoor accepted a position with the University of Minnesota Medical Center and Fairview to develop pharmacy services for chronic pain management.
Dr. Ghafoor is a member of the American Pain Society, American Society of Health-System Pharmacists, and Minnesota Society of Health-System Pharmacists. She has a strong interest in teaching. During the year, she provides numerous didactic lectures and precepts pharmacy students and residents. She has numerous publications in peer reviewed journals and textbooks. Her practice interests involve advocating for pain medication safety, promoting inpatient pain services in community hospitals, and developing an innovative model for mid-level practitioner status for pharmacists specializing in chronic pain management and palliative care.

Phyllis Grauer
Dr. Herndon received both his BS and PharmD from St. Louis College of Pharmacy. Following completion of a geriatrics residency, he was on faculty with the Texas Tech School of Pharmacy where he worked in pain and palliative care with local hospices and a regional cancer center. Prior to his current position, he was a science and research liaison in analgesia with Johnson and Johnson. He is currently the Network Facilitator for Pain within ASHP and serves on the ASHP Task Force of Pain and Symptom Management.

Dr. Nesbit is currently on the Pain & Palliative Care Service at the Sidney Kimmel Comprehensive Cancer Center at The Johns Hopkins Hospital and Clinical Specialist in Pain Management with the Department of Pharmacy. She has been practicing in pain management for over 10 years. Dr. Nesbit has been at The Johns Hopkins Hospital over 7 years. During that time, she has participated in several research protocols and health systems pain initiatives. Along with Dr Stuart Grossman, she developed the Hopkins Opioid Program that is a PDA and web-based application that performs opioid equianalgesic conversions. Dr. Nesbit has served as Chair of the Pain Management Subcommittee of the Maryland Cancer Plan and is currently the Chair of the Pain & Palliative Care PRN of ACCP (American College of Clinical Pharmacy), Co-Chair of the ASHP (American Society of Health-System Pharmacists) Pain and Palliative Care Taskforce, and a Board member of the Maryland Pain Initiative.

Jennifer Strickland received her PharmD from the University of Florida College of Pharmacy and subsequently completed a specialty pharmacy residency in pain management at the H. Lee Moffitt Cancer Center in Tampa, FL. Following her residency, she was the clinical pharmacist for the Pain and Palliative Care Service at the H. Lee Moffitt Cancer Center for 6 years. Following Moffitt, she partnered with a physician and another pharmacist to open an interdisciplinary pain management, addiction, and psychiatry clinic in Tampa, where she served as the clinical coordinator. She currently is the pain management and palliative care pharmacist and director of pharmacy at Lakeland Regional Medical Center in Lakeland, FL. Jennifer is the past-president for the Florida Pain Initiative and is an invited member of the Palliative Care Task Force for the American Society of Health System Pharmacists. She has served as a residency director for a Specialty Pharmacy Residency in Pain management and Palliative Care LRMC and is an adjunct faculty member at University of Florida, FAMU, Palm Beach Atlantic, and Nova Southeastern Colleges of Pharmacy. Jennifer has published in the area of pain management and palliative care and has lectured nationally on these topics as well.

Workshop Fee: $210
Pre-registration is required; Registration must received on or before November 22.
Registration is limited to 60 participants. Lunch is on your own.
In this workshop, learn to build a transitional care plan with patients in various practice settings such as the emergency department, inpatient perioperative setting, subacute rehabilitation, outpatient pain clinic services, cancer center, and palliative care. Using patient cases, this interactive workshop will focus on complex pain management issues involving persistent pain, comorbid physical conditions, substance abuse, multimodal treatment, opioid dosing and medication safety. Using patient-centered care models demonstrating best practice, evidence-based strategies for pharmacotherapeutic decisions, and utilizing quality assurance measurements for assessment of outcomes.
- Assess patient to identify comorbid conditions that complicate the management of pain and other symptoms.
- Design the treatment plan to optimize outcomes and incorporate evidence-based guidelines for medication dosing and side effect monitoring.
- Develop a model for communicating the pain treatment plan across multiple practice settings including the medical home with involvement of the patient and family.
- Identify strategies to prevent adverse events from opioid therapy and manage risk associated with controlled substances.
