Patricia C. Kienle, RPh, MPA, FASHP is Director of Accreditation and Medication Safety for Cardinal Healths Center for Safety and Clinical Excellence. Patricia received a BS in Pharmacy from the Philadelphia College of Pharmacy and Science and an MPA in Health Services Administration from Marywood University in Pennsylvania. Her current responsibilities include assisting client hospitals in implementing best practices that promote safe medication use and continuous compliance with Joint Commission standards. She is a contributing author to Assuring Continuous Compliance with Joint Commission Standards: A Pharmacy Guide, Seventh Edition which is published by the American Society of Health-System Pharmacists.

Lee B. Murdaugh, RPh, PhD, is a member of the Center for Safety and Clinical Excellence at Cardinal Health. Her current responsibilities include assisting client hospitals in implementing best practices that promote safe medication use and continuous compliance with Joint Commission standards. Lee received a BS in Pharmacy and a PhD in Drug and Materials Toxicology from the University of Tennessee and has been a hospital pharmacist since 1982. She is the author of Competence Assessment Tools for Health System Pharmacies and a contributing author to Assuring Continuous Compliance with Joint Commission Standards, published by the American Society of Health-System Pharmacists.
