Michael P. Gulseth, Pharm.D., BCPS is Program Director for Anticoagulation Services at the Sanford University of South Dakota Medical Center, Department of Pharmaceutical Services in Sioux Falls, South Dakota. Dr. Gulseth received his Doctor of Pharmacy degree in 1999 from North Dakota State University in Fargo, North Dakota. He completed residency training at United Hospital in St. Paul, Minnesota. He is lead author and editor of Managing Anticoagulation Patients in the Hospital: the Inpatient Anticoagulation Service, published in 2007 by the American Society of Health-System Pharmacists.
Dr. Gulseth is a member of the Editorial Advisory Board of the American Journal of Health-System Pharmacy. Dr. Gulseth is an active member of several professional pharmacy organizations, including the American Society of Health-System Pharmacists, the Minnesota and South Dakota Societies of Health-System Pharmacists, and the American College of Clinical Pharmacy. His research interests include evaluation of antithrombotic therapies in the inpatient setting and evaluation of the benefits of pharmacist-managed inpatient anticoagulation services.

Michael Crouch,
PharmD, BCPS, Professor and Chair,
South University School of Pharmacy
Dr. Michael Crouch is Professor and Chair of the Department of Pharmacy Practice at South University School of Pharmacy in Savannah, Georgia. He received his Bachelor of Science degree in Pharmacy from the University of North Carolina and his Doctor of Pharmacy degree from the Medical University of South Carolina. He also completed two years of postgraduate training, including a Pharmacy Practice Residency at Wake Forest University Baptist Medical Center and a Medicine/Cardiology Residency at the Medical University of South Carolina.
Dr. Crouch has dedicated much of his career to educating pharmacy trainees, and he has received numerous teaching awards related to these activities. In addition to didactic and experiential teaching, he has developed and supported postgraduate training, which includes serving as the director of a Postgraduate Year Two (PGY2) Cardiovascular Pharmacy Residency program.
Cardiovascular research interests focus on late phase translational research, pharmacoepidemiology, and pharmacoeconomics. Dr. Crouch has published extensively in these areas, including various journal articles and textbook chapters. Additionally, he is the textbook editor for Cardiovascular Pharmacotherapy: a Point-of-Care Guide. He serves on a number of editorial and scholarly boards, including a current appointment on the editorial advisory board of the Annals of Pharmacotherapy. He has practiced in a variety of inpatient and outpatient cardiology settings, including the Coronary Care Unit. Nationally, Dr. Crouch provides many invited presentations related to cardiovascular pharmacotherapeutics. He is a board-certified pharmacotherapy specialist (with added qualifications in cardiology) and a Fellow of the American Society of Health-System Pharmacists.

Edith Nutescu,
PharmD, FCCP, Clinical Professor and Director, Antithrombosis Center,
University of Illinois at Chicago
Edith A. Nutescu, Pharm.D., FCCP, is Clinical Professor of Pharmacy Practice and Director of the Antithrombosis Center at the University of Illinois at Chicago College of Pharmacy and Medical Center. Dr. Nutescu is also part of the Faculty at the University of Illinois at Chicago, Center for Pharmacoeconomic Research. She earned her Pharm.D. degree with high honors at the University of Illinois at Chicago College of Pharmacy. After graduation, Dr Nutescu went on to complete an American Society of Health-System Pharmacists (ASHP) accredited Pharmacy Practice Residency at Lutheran General Hospital Advocate Health Care and a Primary Care Specialty Residency at the University of Illinois at Chicago Medical Center.
As a clinician and educator, Dr. Nutescu has contributed extensively to the care of patients and the education of students and health care providers on topics related to cardiovascular therapeutics and thrombosis. Dr. Nutescu maintains an active clinical practice and research program. Her research and practice interests are in the areas of thrombosis, antithrombotic therapy, cardiovascular diseases, and stroke. Dr Nutescu has authored or co-authored over 100 scientific articles, book chapters, and abstracts published in the science and medical literature and has served as a reviewer for the literature in her field. She serves on the Editorial Boards for Annals of Pharmacotherapy and the American Journal of Health-System Pharmacy and serves as the Vice-President and on the Board of Directors for the Anticoagulation Forum.

Ann Wittkowsky,

Ann K. Wittkowsky, PharmD, CACP, FASHP, FCCP is Clinical Professor of Pharmacy at the University of Washington School of Pharmacy and Director of Anticoagulation Services at the University of Washington Medical Center. As a clinician and educator in the field of antithrombotic pharmacotherapy, she has contributed extensively to the care of patients and the education of health care providers regarding antithrombotic pharmacotherapy.
She is the author of over 100 scientific papers, book chapters and abstracts, and co-editor of the textbook Managing Oral Anticoagulation Therapy: Clinical and Operational Guidelines. She has lectured widely throughout the United States and Canada, maintains an active clinical research program, and is board certified as an anticoagulation care provider. Dr. Wittkowsky serves as an editorial board member for Pharmacotherapy and the Journal of Thrombosis and Thrombolysis, as a reviewer for numerous medical and scientific journals, and as a member of the board of directors of The Anticoagulation Forum, a multidisciplinary organization of anticoagulation care providers. She is the director of the Northwest Anticoagulation Consortium, a preceptor for the ASHP Research and Education Foundations Antithrombotic Pharmacotherapy Traineeship, a member of the Board of Regents of the American College of Clinical Pharmacy, and of the Board of Directors of Pharmacotherapy. Dr Wittkowsky was a member of the American College of Chest Physicians 8th Conference on Antithrombotic and Thrombolytic Therapy and was recently selected to serve as an author for the 9th edition of these guidelines.
