RPh, MSc, Medication Systems Consultant and Clinical Associate Professor of Pharmacy Practice,
South Carolina College of Pharmacy
Toby Clark, R.Ph., M.Sc, FASHP is an adjunct faculty member at the South Carolina College of Pharmacy, and formerly Director, Pharmacy and adjunct Professor at University of Illinois at Chicago, College of Pharmacy. He currently services a Residency Accreditation Team Leader for ASHP. Active in international pharmacy, he has lectured in over 10 countries and serves as Treasurer, FIP Hospital Pharmacy Section. He is a founding member of the Oncology BPS, Novation Pharmacy Executive Committee, and is a past chair of UHC Pharmacy Council and Executive Committee. Toby has a strong interest in residency training, leadership, building future leaders and practioners, and medication system informatics and safety.

Mr. Michael Flagstad, MS, RPh, President, Visante, Inc.
Dr Neil MacKinnon is Professor and Associate Director for Research at the Dalhousie University College of Pharmacy at Halifax, Nova Scotia, Canada. He is also cross-appointed as a Professor in the Faculty of Medicine and the School of Health Administration.
Neil completed his BSc (Pharmacy) at Dalhousie University, a Master's degree in Hospital Pharmacy and administrative residency at the University of Wisconsin and a PhD in Pharmacy Health Care Administration and Fellowship at the University of Florida.
Neil is currently president-elect of the Canadian Society of Hospital Pharmacists.

Amber Meigs is a Clinical Pharmacist at Evanston Hospital. She is also the Medication Safety Officer for NorthShore University HealthSystem. She received her Doctor of Pharmacy degree from Drake University College of Pharmacy in 2004 and completed a PGY1 Pharmacy Practice Residency at Evanston Hospital in Evanston, IL.
