Deb Swartwood Ash is currently President and C.E.O. of Ash Pharmacy Consultants. She provides consulting in all aspects of Pharmacy and other areas of healthcare (disease management, case management, population-based data analysis and program development)in both the United States and Canada. Deb specializes in Medicare and senior populations. She also provides professional education program coordination as well as professional publication editing. Previously Ms. Ash worked in numerous positions at Blue Cross Blue Shield of Florida as well as Pharmacy Gold, Inc., the PBM subsidiary of Blue Cross Blue Shield of Minnesota. Deb has also served as Executive Director for the Minnesota Society of Health-System Pharmacists, Director of Pharmacy at Memorial Medical Center in Ashland, Wisconsin and Supervisor of Intravenous Admixture Services at the University of Minnesota Hospitals and Clinics.
Deb received her Master of Science in Hospital Pharmacy from the University of Minnesota and a Bachelor of Science in Pharmacy from South Dakota State University. She has completed the Wharton Executive Development Program at the University of Pennsylvania. Deb has held leadership roles in many professional organizations as well as participated in Leadership Miami. Deb received the Meritorious Service Award from the Minnesota Society of Health-System Pharmacists and was named in Women in Pharmacy Leadership as well as Who's Who in Executives and Professionals.

Michelle Allen,

Michelle E. Allen, PharmD, BCPS, is the PGY1 Residency Director and the Clinical Coordinator for Sibley Memorial Hospital. She received her Bachelors of Science degree from the University of California at Berkeley, and her Doctor of Pharmacy degree from the University of Pittsburgh. Residency training was completed at the University Medical Center at the University of Arizona in Tucson, AZ; there Michelle completed a PGY1 residency, as well as a PGY2 in Critical Care. Dr. Allen was a clinical instructor for the University of Arizona's College of Pharmacy and continues to be an adjunct Clinical Associate Professor for the University of Florida's College of Pharmacy. Michelle is active professionally in both pharmacy and critical care, having served as a Chair for both ASHP's New Practitioners Forum, and the Society of Critical Care Medicine's (SCCM) Education and Training Steering Committee. She is a member of SCCM's National Research Committee, as well as a member of ASHP, the American College of Clinical Pharmacy, and the Washington Metropolitan Society of Health-System Pharmacists. Dr. Allen is a past recipient of the Presidential Citation from SCCM, a published author, and has presented Nationally and Internationally.

Juliana Chan, Pharm.D. is Clinical Assistant Professor in the Department of Pharmacy Practice at the University of Illinois at Chicago (UIC) College of Pharmacy and in the Section of Digestive Diseases and Nutrition and Section of Hepatology at the UIC College of Medicine. She is the Assistant Director of Pharmacy Clinical Services and a Clinical Pharmacist in the Liver Center at the UIC Outpatient Care Center. Dr. Chan obtained her Bachelor of Science in Pharmacy and Doctor of Pharmacy degrees from Massachusetts College of Pharmacy. She then completed a Pharmacy Practice Residency and a Specialized Residency in Gastroenterology and Hepatology at University of Michigan. At UIC, she teaches and conducts research in gastroenterology and hepatology, focusing her research efforts in the area of viral hepatitis, drug-induced liver disease as well as outcomes research with proton pump inhibitor therapy. Over the past year, she has focus her research efforts in pharmacy administration.

Toby Clark,

Toby Clark, R.Ph., M.Sc, FASHP is an adjunct faculty member at the South Carolina College of Pharmacy, and formerly Director, Pharmacy and adjunct Professor at University of Illinois at Chicago, College of Pharmacy. He currently services a Residency Accreditation Team Leader for ASHP. Active in international pharmacy, he has lectured in over 10 countries and serves as Treasurer, FIP Hospital Pharmacy Section. He is a founding member of the Oncology BPS, Novation Pharmacy Executive Committee, and is a past chair of UHC Pharmacy Council and Executive Committee. Toby has a strong interest in residency training, leadership, building future leaders and practioners, and medication system informatics and safety.

Darrell Contreras,

Darrell is the Chief Compliance Officer for Lakeland Regional Medical Center in Lakeland, Florida. In this capacity, Darrell is responsible for oversight and operation of the Compliance and Privacy Programs as well as the organizations Internal Audit Program. Darrell relies on his expertise and experience in compliance to focus on creating and demonstrating effectiveness in the Compliance and Internal Audit functions as set forth in guidance from the Office of Inspector General of the Department of Health and Human Services and the Institute of Internal Auditors, respectively. Prior to becoming a Chief Compliance Officer, Darrell was a consultant with Ernst & Youngs Health Science Advisory Services practice where he assisted health care organizations throughout the United States with the development, implementation and assessment of their Compliance and HIPAA Privacy Programs. During this time, Darrell also assisted clients that were under a Corporate Integrity Agreement while serving in the Independent Review Organization function.
An accomplished speaker, Darrell has spoken on compliance and privacy related topics to the Health Care Compliance Associations Annual Compliance Annual Compliance Institute. Darrell has also published compliance-related articles in the Journal of Health Care Compliance and the Compliance Today.
Darrell holds a Juris Doctor degree from the University of Southern California and is a member of the State Bar of Arizona. He has a Bachelor of Arts degree in Political Science from the University of Southern California. Darrell is also Certified in Healthcare Compliance and Certified in Healthcare Research Compliance through the Healthcare Compliance Certification Board.

Debra S. Devereaux MBA, R.Ph., FASHP is a Senior Consultant for the Gorman Health Group LLC, a Washington, D.C. based Medicare consulting firm. She is a project manager specializing in Medicare Part D Compliance, PBM Compliance, Care Management/Disease Management for chronic conditions, behavioral health, frail elderly, and dual eligibles and Special Needs Plans.
Previously, she was an adjunct clinical professor and served as DUR Manager for Wyoming Medicaid at the University of Wyoming School of Pharmacy in Laramie, Wy. She served as liaison to the Wyoming Department of Health, administered the Wyoming Drug Utilization Review Board, edited the newsletter, conducted population based research on diseases and chronic care and presented educational programming for physicians, pharmacists, nurses and other health care providers in Wyoming
Debra began her pharmacy career as Assistant Director of Pharmacy at University Hospital in Denver, Colorado where she served on the UCHSC Institutional Review Board, the Nutrition Support team and had administrative responsibility for the clinical pharmacy programs.
Ms. Devereaux was the President of ASHP during 2002-2003 after serving on the ASHP Board of Directors for three years. She has served on numerous councils, commissions and committees for ASHP including Vice Chair of the ASHP Research and Education Foundation and Chair of the ASHP Commission on Goals. She served as Secretary of the American Drug Utilization Review Society.

Matthew Eberts earned a PharmD from Duquesne Univeristy in Pittsburgh, PA and an MBA from Robert Morris University also in Pittsburgh, PA. He is empoyeed by the Univeristy of Pittsburgh Medical Center as Manager of Pharmacy Operations at the Presbyterian Hospital campus. UPMC Presbyterian is a 750 bed tertiary care teaching hospital. Matt serves as a preceptor for both the Pharmacy Administrative Residents and Pharmacy Practice Residents. He lectures at the University of Pittsburgh School of Pharmacy. Matt has served as President of the Pennsylvania Society of Health System Pharmacist and as a SAG member for various groups in the ASHP Practice Management Section.

Staci Hermann, M.S., Pharm.D., graduated from the University of Rhode Island in 2004 with her Pharm.D. degree. In 2007, she received a M.S. in Hospital Pharmacy from The University of Wisconsin. While finishing her graduate work, she completed a PGY-2 health- system administration residency at The University of Wisconsin Hospital and Clinics. Staci is actively involved with numerous organizations such as ASHP, Greater Kansas City Society of Hospital Pharmacists (GKSHP), and the University HealthSystem Consortium (UHC). She has worked on various committees within these organizations such as the ASHP Section of Pharmacy Managers Programming Committee and the UHC Medication Use Informatics & Technology Committee. She currently serves as Pharmacy Operations Manager at the University of Kansas Hospital. Her responsibilities include management of 40+ technicians and pharmacists, IV Room/ Sterile Products, daily operations, as well as all the pharmacy automation and technology. In addition, Staci is actively involved in optimizing the interface between the pharmacy information system and the automation/ technology and serving as a core preceptor in the PGY2 Management Residency Program. Her interest areas include staff development, quality improvement, informatics, and technology optimization.

Dave Hicks,

David W. Hicks, R.Ph., M.B.A., received his B.S. in Pharmacy from the University of Illinois and M.B.A. from the Keller Graduate School of Management. He has served as Vice-President and Chief Pharmacy Officer at University of Chicago Medical Center since 2007, where he has engineered a department turnaround, with significant cost savings, service improvements, new clinical initiatives, and the starting of a pharmacy residency program. He was previously Director of Automation for McKesson Medication Management, with responsibilities for operational redesigns and optimizing pharmacy automation systems at 150 hospital pharmacies managed nationwide by McKesson Medication Management. Mr. Hicks has also provided consulting support for McKesson customers such as Advocate Health Care, University of Chicago, Sutter Health, University of Michigan, New York Presbyterian Health System, Hospital Corporation of America, and River Valley Health Region in Canada.
He joined McKesson in 2000 after five years at Rush-Copley Medical Center in Aurora, Illinois as Director of Pharmacy and Information Systems. As Rush-Copley, he led the selection and implementation of a hospital-wide electronic medical record/physician order-entry system as well as approximately thirty other clinical and financial systems. He was Director of Pharmacy at Northern Illinois Medical Center from 1984 to 1996 where he led major, organization-wide initiatives in re engineering, operational improvement, ambulatory services, and implemented the first hospital-wide profiled unit-based cabinet dispensing system in the state of Illinois. From 1991 to 1993 he also served as Director of Purchasing and Materials Management.

Tricia Killingsworth is the Pharmacy Director of Clinical Services for Catholic Health Initiatives (CHI). She has worked in the pharmacy profession for more than 25 years beginning as a pharmacy assistant, graduating with her B.S. in Pharmacy from the University of Washington in 1993, and completing two ASHP accredited pharmacy residency programs. Her management experience includes Regional Ambulatory Pharmaceutical Services Manager with Franciscan Health System in Tacoma, Washington, Director of Pharmacy Services at Mercy Medical Center in Nampa, Idaho, Executive Director of the Idaho Society of Health-System Pharmacists, in Boise and Idaho and Pharmacy Compliance Officer for MedNow, Inc. in Nampa, ID.
Tricia is actively involved in pharmacy volunteer work. She is currently serving on the Idaho Society of Health System-Pharmacists Education Committee, Board of Directors, and Executive Committee and was elected as the Director at Large for the American Society of Health-System Pharmacists Section of Pharmacy Practice Managers in 2008.
Tricia has previously presented at local, state and national meetings on various topics including Clinical Pharmacy Productivity, USP <797>, Pharmacy Knowledge Communities, Medication Reconciliation Programs, and Surviving and Thriving in Pharmacy during tough economic times.

Scott Knoer,

Scott J. Knoer, M.S., Pharm.D., is the Director of Pharmacy at the University of Minnesota Medical Center, Fairview. He holds academic appointments at the University of Minnesota College of Pharmacy and the Graduate School in Social and Administrative Pharmacy where he teaches the Institutional Pharmacy class and the Hospital Pharmacy course respectively. Knoer received his B.A. from Creighton University, his Pharm.D. from the University of Nebraska and completed a combined residency and M.S. in Hospital Pharmacy at the University of Kansas. Knoer has served in the ASHP House of Delegates (2005-2006), as Chair of the Section Advisory Group on Leadership Development (2004-2006), as a member of ASHPs Commission on Affiliate Relations (2006-2007) and as Director-at-Large for the ASHP Practice Managers Section (2007 - 2009. He also serves as Chair of the University Health-System Consortium (UHC) Process Improvement and Compliance Council. Knoer has also served in variety of local and state affiliate roles in Texas and Minnesota.
Dr. Knoer is co-creator of the ASHP Managers Bootcamp, which is offered annually at the ASHP Leadership Conference. He has multiple publications and is a frequent invited lecturer on a variety of health-care related topics.

Chris Lowe is the Assistant Director of Clinical Services at Novant Health. Dr. Lowe received his Doctor of Pharmacy degree from the University of Pittsburgh, and completed both a pharmacy practice and an oncology specialty residency at The Johns Hopkins Hospital in Baltimore, MD. His current practice is focused on defining and implementing core clinical pharmacy services throughout the Triad Region Facilities and managing the Regional P&T Committee. Before joining Novant, Dr. Lowe maintained a clinical practice with the Bone Marrow Transplant team and managed the Taussig Cancer Center Pharmacy at the Cleveland Clinic in Cleveland, Ohio.

Dr Neil MacKinnon is Professor and Associate Director for Research at the Dalhousie University College of Pharmacy at Halifax, Nova Scotia, Canada. He is also cross-appointed as a Professor in the Faculty of Medicine and the School of Health Administration.
Neil completed his BSc (Pharmacy) at Dalhousie University, a Master's degree in Hospital Pharmacy and administrative residency at the University of Wisconsin and a PhD in Pharmacy Health Care Administration and Fellowship at the University of Florida.
Neil is currently president-elect of the Canadian Society of Hospital Pharmacists.

Marc Summerfield is the Director of Pharmacy at the University of Maryland Medical Center in Baltimore. He also serves as Adjunct Professor of Pharmacy at the School of Pharmacy.
Marc received his Bachelors degree in Pharmacy at the University of Connecticut in 1974. In 1976, Marc completed a residency in hospital pharmacy at the Johns Hopkins Hospital and his Masters degree in Hospital Pharmacy at the University of Maryland.

Bruce received his BS in Pharmacy at Northeastern University and his MS in Pharmacy at the University of Florida. He completed his University of Florida Clinical Pharmacy Residency at Shands Teaching Hospital. Following residency completion he worked as a Clinical Pharmacy Specialist in Cardiothoracic Surgery at Tufts-New England Medical Center in Boston, MA. Bruce has also served as Assistant Director for Clinical Pharmacy Services at Saint Elizabeth's Medical Center in Boston, MA. He has also served as faculty at Tufts University School of Medicine, Northeastern University, as well as Massachusetts College of Pharmacy, all in Boston, MA. Bruce currently serves as Vice President - Pharmacy for VHA. With his team of VHA Pharmacy Implementation Managers, Bruce works with the members of VHA to promote cost effective medication therapies.

Sara J. White, MS and FASHP, is currently a Pharmacy Leadership Coach. In 2004 she was an ASHP Foundation Scholar in Residence on Pharmacy Leadership. From 1992 to 2003 Sara was Stanford Hospital and Clinics, Director of Pharmacy and Clinical Professor, UCSF School of Pharmacy. She was Associate Director of Pharmacy and Professor at the University of Kansas Medical Center from 1972-1992. Sara completed a BS in Pharmacy from Oregon State University, MS from Ohio State University and an ASHP accredited Residency at the Ohio State University Hospitals and Clinics. She has served as ASHP president in 1996 and received the Harvey A.K. Whitney Award in 2006.

Jerome Wohleb,

Jerome Wohleb is the administrative pharmacy director for the Urban South Region based in Utah at Utah Valley Regional Medical Center in Provo, a non-profit hospital owned by Intermountain Healthcare. He has served in this role since 2004 and been a pharmacist for 30 years in a variety of practice settings. Dr. Wohleb is responsible for overseeing three hospitals in the Intermountain system and led several pharmacy related legislative efforts on a local and national level.
Before joining the Utah hospital system, Dr. Wohleb was the pharmacy director at Maricopa Integrated Health System in Phoenix, Arizona. During this period, he participated on both the Arizona Society Health-System Pharmacists Board and the Arizona Pharmacy Association. He has held several positions in the Arizona Society, including president.
Wohleb is passionate about clinical pharmacy and would like to position pharmacy practice in the center of healthcare reform.
Dr. Wohleb received his MBA from the University of Utah in May 2009, and his Pharm.D. from the University of Nebraska in 1979.

Glenn Wysock,

Glenn graduated with a BS from Mercer University Southern School of Pharmacy in June 1980. Glenn began work with Winter Haven Hospital as a staff pharmacist in 1988. In 1990 he developed and implemented decentralized pharmacy services and in subsequent years opened the ICU decentralized services as well as an OR Pharmacy satellite. In 1995 Glenn was promoted to Assistant Director and in 2000 to Director of Pharmacy.
With Glenn as Director of Pharmacy, Winter Haven Hospital implemented a cabinet based automated drug distribution system, upgraded the Pharmacy Information System to one that includes a rules based engine and lab interface, expanded clinical pharmacy services focusing on medication safety as well as the latest practice guidelines and the pharmacy recently moved into a new 7000 square foot department.
