As operations manager at a JC accredited hospital, we will be required to meet the anticoagualtion assesment/treatment goals for our patients. Currently there are several good programs in place around the country. This program will highlight the JC patient safety goals and intent, review practices currently in place, and provide key information to inpatient care practitioners regarding anticoagualtion and communication across inpatient and out patient settings to include small and rural hospitals and clinics.

Gary is the Clinical Pharmacy Manager for Iowa Health – Des Moines which includes Iowa Methodist Medical Center, Iowa Lutheran Hospital, Blank Children’s Hospital and the Partners in Health Clinics. He is also an Adjunct Assistant Professor for Drake University’s College of Pharmacy and is a preceptor for the IH-DM hospitals rotation students.
Gary is a graduate of Iowa State University, Ames, Iowa, receiving a Bachelor’s degree in Psychology in 1981, and a graduate of Drake University, Des Moines, Iowa, receiving a Bachelor’s degree in Pharmacy in 1990, and a Doctor of Pharmacy in 1997.
Dr Lori L Schirmer is an Assistant Professor, Department of Pharmacy Practice, College of Pharmacy and Health Sciences, at Drake University in Des Moines, Iowa. She is Associate Faculty, Department of Surgery Education, and Clinical Pharmacist for Iowa Heath System in Des Moines, Iowa. She is a 1994 graduate of Maryville College with a BA in Chemistry and a 1996 graduate of The University of Texas at Austin with a MA in Biochemistry. She earned her Pharm.D. in 2003 from The University of Tennessee, College of Pharmacy and completed a PGY1 residency at the VA Medical Center in Memphis, TN. PGY2 residency training in Critical Care and Nutrition Support was completed at The University of Tennessee at Memphis and the Regional Medical Center at Memphis’ Elvis Presley Memorial Trauma Center. She is a board certified pharmacotherapy specialist (BCPS). Her teaching, research, and patient care activities support the trauma and surgical patient populations at Iowa Methodist Medical Center in Des Moines, Iowa.

Explain how to develop teamwork skills among providers to enhance communication and ultimately a safer patient environment.
Identify relative risks associated with anticoagulant medications.
