Tracy Martinez, PharmD, Clinical Pharmacy Specialist, Henry Ford Health System
Currently working in ambulatory care setting, involved with lipid management, asthma management, diabetes management and medication therapy management for senior patients. Elderly patients make up a large percentage of my patient pool and have allwed me to become familiar with the particular situations facing elderly patients, especially in terms of polypharmacy.

Judith L. Beizer, PharmD, CGP, FASCP
Dr Beizer is a Clinical Professor in the Department of Clinical Pharmacy Practice at St John's University College of Pharmacy and Allied Health Professions. As part of her teaching responsibilities, she serves as the Clinical Pharmacist at the Center for Extended Care and Rehabilitation, a long-term care facility in Manhasset, NY. At this facility, Dr Beizer has expanded clinical pharmacy services and precepts pharmacy students.
Dr Beizer received her Bachelor of Science in Pharmacy from the St Louis College of Pharmacy and then earned a Doctor of Pharmacy degree from the University of Tennessee. After pursuing a residency in clinical pharmacy at the Hospital of the University of Pennsylvania, she completed a fellowship in geriatric pharmacy at Montefiore Medical Center, Bronx, NY. During her fellowship, Dr Beizer was involved in a National Institute on Aging (NIA) grant concerning medication use and pharmacist intervention in community-dwelling elderly
Dr Beizer is a member of a number of professional organizations, including the American Society of Health-System Pharmacists (ASHP), American Society of Consultant Pharmacists (ASCP), American College of Clinical Pharmacy (ACCP), American Pharmacists Association (APhA), and the American Geriatrics Society (AGS). She currently serves as the President-Elect of ASCP.
Dr Beizer regularly speaks on the topic of medication use in the elderly and has published articles and abstracts on various issues in geriatric pharmacotherapy. She is a co-author of the publication, Geriatric Dosage Handbook.
Sandra Chase, Clinical Specialist for Cardiology, Spectrum Health Hospitals
Dr. Chase is currently a Cardiopulmonary Clinical Specialist seeing patients in a specialty cardiology clinic affiliated with the health system. She also works on the quality teams for Heart Failure, AMI, and VTE prophylaxis. She is past president of the Michigan Pharmacists Association and currently chairs the Education Committee of the Michigan Society of Health-System Pharmacists. She is a member of the Michigan Pharmacy Foundation Executive Board and the University of Michigan Alumni Counsel. She is a past member of the ASHP Section of Home, Ambulatory, and Chronic Care executive board.
