William Dager,

One of the major areas of patient care responsibilities for me has been the management of anticoagulation therapy, primarily in the inpatient setting. This has resulted in dozen of published manuscripts covering a wide number of related issues. I have also represented pharmacy on multi-disciplinary national events, and lectured on optimizing anticoagulation therapy. My practice setting has been highlighted in AJHP and by ACCP. Other related activities include serving as the chair of the editorial advisory board for The Annals of Pharmacotherapy panel on anticoagulation, and authored three chapters in the ASHP textbook on anticoagulation therapy edited by Michael Gulseth.

Robert Gosselin the technical coagulation specialist in the Department of Clinical Pathology and Laboratory Medicine at UC Davis Health System. He has been performing specialized coagulation testing for over 20 years and has published numerous articles related to hemostasis and quality assurance in coagulation testing.

Dr. Grice is an Assistant Professor at the St. Louis College of Pharmacy. She graduated from the University of Maryland, School of Pharmacy in 2002. She went to St. Louis to complete a residency in family medicine at the St. Louis College of Pharmacy and St. John's Mercy Family Medicine. After 4 years in family medicine, she changed her practice to specialize in pharmacogenetic-guided anticoagulation. She has been doing clinical research and patient care in this area at Washington University/Barnes Jewish Hospital with Drs. Brian Gage and Dr. Paul Milligan for the past two years. Her work with the anticoagulation service has primarily focused on orthopedic patients, but in the past year, it has grown to include inpatients with other indications for VTE treatment or prevention.

Ann Wittkowsky,

Ann K. Wittkowsky, PharmD, CACP, FASHP, FCCP is Clinical Professor of Pharmacy at the University of Washington School of Pharmacy and Director of Anticoagulation Services at the University of Washington Medical Center. As a clinician and educator in the field of antithrombotic pharmacotherapy, she has contributed extensively to the care of patients and the education of health care providers regarding antithrombotic pharmacotherapy. She is the author of over 100 scientific papers, book chapters and abstracts, and co-editor of the textbook Managing Oral Anticoagulation Therapy: Clinical and Operational Guidelines. She has lectured widely throughout the United States and Canada, maintains an active clinical research program, and is board certified as an anticoagulation care provider. Dr. Wittkowsky serves as an editorial board member for Pharmacotherapy and the Journal of Thrombosis and Thrombolysis, as a reviewer for numerous medical and scientific journals, and as a member of the board of directors of The Anticoagulation Forum, a multidisciplinary organization of anticoagulation care providers. She is the director of the Northwest Anticoagulation Consortium, a preceptor for the ASHP Research and Education Foundation’s Antithrombotic Pharmacotherapy Traineeship, and a member of the American College of Chest Physician’s 8th Conference on Antithrombotic and Thrombolytic Therapy. She was recently elected to the Board of Regents of the American College of Clinical Pharmacy.
