Dr. Chanel Agness, PharmD, BCPS, Assistant Professor, University of Maryland School of Pharmacy, Department of Pharmacy Practice and Science
Chanel Agness is an Assistant Professor at the University of Maryland, School of Pharmacy. She is responsible for teaching various didactic lectures to Doctor of Pharmacy candidates and serves as a faculty preceptor during the experiential phases of the curriculum. Current practice sites are in ambulatory care clinical settings working with seniors, and research interests include chronic kidney disease and health literacy in the elderly population. She received her BS in Pharmacy and Doctor of Pharmacy from Rutgers University in New Brunswick, New Jersey in 2003. She completed a managed care pharmacy practice residency at Kaiser Permanente in 2004 and a primary care specialty residency at the Baltimore Veterans Affairs in 2005. She was credentialed as a Board Certified Pharmacotherapy Specialist in 2005. |

Biographical Sketch
Jannet M. Carmichael
Jannet M. Carmichael, Pharm.D., FCCP, BCPS is the VISN 21 Pharmacy Executive for the VA Sierra Pacific Network in Northern Nevada, Northern California and Hawaii. Dr. Carmichael coordinates a Health Informatics, Pharmacoeconomic and Pharmacoepidemiology program using a Drug Data Warehouse, PBM performance measures and therapeutic outcomes for the Veteran Integrated Service Network (VISN 21). Prior to this she was Clinical Pharmacy Coordinator at the VA Sierra Nevada Health Care System in Reno, Nevada and Associate Professor of Medicine at the University of Nevada School of Medicine. Her clinical practice includes primary care and outcomes studies. Dr. Carmichael coordinated pharmacy specialists, students and residents, as well as therapeutic research for VISN activities in the health system. Jan also serves as a Pharmacy Residency Program Director for a PGY2 program in Managed Care & Pharmacoeconomics.
Prior to coming to Reno, Dr. Carmichael taught at the University of Iowa, College of Pharmacy and the University of North Carolina, College of Pharmacy. Dr. Carmichael is a graduate of the University of Iowa College of Pharmacy and the University of the Pacific College of Pharmacy Pharm.D. program. In addition, Jan is Past-President of the American Society of Health-system Pharmacists, past member of the Nevada Board of Pharmacy, Past-President of the Nevada Pharmacists Association and of the Nevada Pharmacy Alliance. She continues active pharmacy participation as a member of numerous professional boards and committees. She currently serves as Chair of the Board of Pharmaceutical Specialties, has Chaired the BCPS Pharmacotherapy Council, has been a member of the APhA Strategic and Tactical Assessment Team on Payment, and is an ACPE Practitioner Reviewer.
Jan has authored many professional papers, chapter and research projects. She is a frequent speaker on primary care, pharmaceutical care and progressive pharmacy practice throughout the country.
Jan resides in Reno, Nevada. She has 2 children, Jenna, 23 and Julia, 20.

Description:
In this interactive workshop, clinical pharmacy managers and services coordinators, pharmacy administrators, clinical pharmacists, and residents can explore the process of implementing an outpatient anemia management program. This session was planned in cooperation with the ASHP Quality Improvement Initiative.
Learning Objectives:
- Explain the clinical implications of establishing an anemia management program.
- Dscribe the process of clinical privileging and developing a scope of practice.
- Identify key quality standards related to anemia management and describe how to incorporate them into your practice.
